【摘要】：目的觀察絕經(jīng)后女性不同Singh指數(shù)股骨頭骨小梁結(jié)構(gòu)及骨超微結(jié)構(gòu)情況。方法取股骨頸骨折、粗隆間骨折絕經(jīng)后女性患者行假體置換患者股骨頭標(biāo)本33份,將33份標(biāo)本采用低真空掃描電鏡觀察股骨頭主要承重區(qū)骨小梁和骨超微結(jié)構(gòu);拍攝術(shù)前健側(cè)X線片進(jìn)行Singh指數(shù)分級(jí)。結(jié)果不同Singh指數(shù)分級(jí)掃描電鏡下呈現(xiàn)出明顯的差異,對(duì)于Singh指數(shù)6級(jí),骨小梁結(jié)構(gòu)完整顯示清晰,高倍鏡下骨膠原纖維排列緊密為特征;Singh指數(shù)5級(jí)和4級(jí)鏡下辨別困難,共同特點(diǎn)為骨小梁變細(xì),表面欠光滑,部分骨小梁出現(xiàn)斷裂和缺失現(xiàn)象,骨膠原纖維出現(xiàn)卷曲,局部出現(xiàn)分散情況,但這對(duì)于6級(jí)和3級(jí)分辨清晰;Singh指數(shù)3級(jí)掃描電鏡下骨小梁斷裂、缺失情況普遍,骨膠原纖維斷裂、排列散亂為其特征;Singh指數(shù)1級(jí)和2級(jí)分辨困難,表現(xiàn)為骨小梁結(jié)構(gòu)破壞嚴(yán)重,骨膠原纖維出現(xiàn)大部分?jǐn)嗔�、缺失情況。結(jié)論女性患者Singh指數(shù)可以大致反映骨結(jié)構(gòu)的變化,了解骨質(zhì)疏松情況,但因其主觀因素影響,6級(jí)分法不能更加準(zhǔn)確反映出骨結(jié)構(gòu),尤其是骨超微結(jié)構(gòu)的具體變化。
[Abstract]:Objective To observe the structure and ultrastructure of the bone trabecular bone of the femur bone with different Singh index in postmenopausal women. Methods the femoral neck fracture, intertrochanteric fractures in postmenopausal women with prosthetic replacement in patients with femoral head specimens of 33 copies, 33 specimens with low vacuum scanning electron microscope observation of the main bearing area of the femoral head trabecular and bone ultrastructure; taken preoperative contralateral radiographs were graded index Singh. The results of different Singh index under the scanning electron microscope showed a significant difference, Singh index for 6, the trabecular structure complete clear display, high magnification of bone collagen fibers arranged closely to feature; Singh index 5 and 4 mirror to distinguish difficult, common characteristics for the bone surface was not smooth, part of Liang Bianxi. Trabecular bone fracture and the phenomenon of lacking collagen fiber appeared curl, the emergence of local dispersion, but for the 6 level and 3 level resolution clear; Level 3 Singh index under scanning electron microscopy, trabecular bone fracture, loss of common faults, the collagenous fibers were scattered as its features; Singh index of grade 1 and 2 the difficulty level resolution, showed that the trabecular bone structure damaged seriously, most of the broken, loss of collagen fiber appeared. Conclusion the Singh index of female patients can reflect the change of bone structure and understand osteoporosis. However, because of its subjective factors, the 6 level method can not reflect the specific changes of bone structure, especially the ultrastructure of bone.
