【摘要】：目的比較單側(cè)與雙側(cè)椎體成形術(shù)(PVP)治療骨質(zhì)疏松性椎體壓縮骨折的臨床效果。方法篩選行PVP治療骨質(zhì)疏松性椎體壓縮骨折的患者62例,根據(jù)術(shù)式分為單側(cè)組和雙側(cè)組,單側(cè)組32例32個(gè)椎體,雙側(cè)組30例30個(gè)椎體。比較兩組手術(shù)時(shí)間、術(shù)中X線曝光次數(shù);術(shù)后疼痛緩解情況;傷椎高度恢復(fù)值及術(shù)后并發(fā)癥。結(jié)果所有患者均順利完成手術(shù),兩組術(shù)后第2天及術(shù)后1年隨訪時(shí)VSA評(píng)分較術(shù)前均明顯改善(P0.05),但兩組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);手術(shù)時(shí)間,X線曝光次數(shù)單側(cè)組少于雙側(cè)組(P0.05),兩組術(shù)后傷椎平均高度較術(shù)前均有顯著恢復(fù)(P0.05),但兩組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);單側(cè)組骨水泥滲漏率9.4%,雙側(cè)組10.0%,兩組差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),兩組均無(wú)骨水泥脫離椎體后緣進(jìn)入神經(jīng)椎管,也未發(fā)現(xiàn)骨水泥滲漏引起的臨床癥狀。結(jié)論經(jīng)皮單側(cè)PVP治療骨質(zhì)疏松性椎體壓縮骨折,術(shù)中掌握好進(jìn)針角度,并采用多次推注的方法,可以達(dá)到與傳統(tǒng)的經(jīng)皮雙側(cè)PVP同樣的臨床效果,且經(jīng)皮單側(cè)PVP還具有手術(shù)時(shí)間短、術(shù)中創(chuàng)傷小、術(shù)中X線輻射劑量少等優(yōu)點(diǎn),值得臨床術(shù)者優(yōu)先選擇。
[Abstract]:Objective to compare the clinical effect of unilateral and bilateral vertebroplasty (PVP) in the treatment of osteoporotic vertebral compression fractures. Methods 62 cases of osteoporotic vertebral compression fractures treated by PVP were selected and divided into unilateral and bilateral groups according to operation. Unilateral group consisted of 32 cases, 32 vertebrae, bilateral group 30 cases, 30 vertebrae. The operation time of the two groups, the number of X-ray exposure in the operation, the relief of pain after the operation, the height recovery of the injured vertebra and the postoperative complications were compared. Results all patients were successfully completed surgery, two group after second days and 1 year follow-up when compared with preoperative VSA score were significantly improved (P0.05), but there was no significant difference between the two groups (P0.05); the operation time, X-ray exposure times were less than unilateral bilateral group (P0.05), the two groups after injury the average height of vertebral than before were recovered significantly (P0.05), but there was no significant difference between the two groups (P0.05 group); unilateral bone cement leakage rate of 9.4%, 10% groups, no significant difference between two groups (P0.05), bone cement from the posterior margin of the vertebral body into the spinal nerve were two groups, found no clinical symptoms caused by the leakage of bone cement. Conclusion percutaneous unilateral PVP treatment of osteoporotic vertebral compression fractures, intraoperative grasp the puncture angle, and the method of repeated injection, can be achieved with the traditional percutaneous bilateral PVP the same clinical effect, and percutaneous unilateral PVP also has the advantages of shorter operative time, less trauma, intraoperative X-ray radiation dose less, it is worthy of clinical surgery is preferred.
