【摘要】：目的探討經(jīng)皮腎鏡碎石術(shù)(percutaneous nephrolithotomy,PCNL)后雙J管留置相關(guān)嚴(yán)重并發(fā)癥的危險因素。方法依標(biāo)準(zhǔn)篩選2014年1月~2016年4月泌尿外科PCNL術(shù)治療上尿路結(jié)石并留置雙J管病例272例,運用SPSS統(tǒng)計軟件對可疑危險因素行二分類單變量Logistics回歸篩選,有統(tǒng)計學(xué)意義之因素再行多變量Logistics回歸逐步分析。結(jié)果共63例患者發(fā)生較嚴(yán)重的雙J管相關(guān)并發(fā)癥(發(fā)生率23.1%),篩選后具有統(tǒng)計學(xué)意義的危險因素包括:輸尿管異常(β=1.735,P=0.000,OR=5.670)、留置時間(β=1.206,P=0.028,OR=3.340)、性別(β=0.895,P=0.016,OR=2.446)、術(shù)前控制欠佳的尿路感染(β=0.849,P=0.020,OR=2.338)和粗細(xì)型號(β=0.847,P=0.011,OR=2.333)。結(jié)論男性是PCNL術(shù)后雙J管嚴(yán)重并發(fā)癥的危險人群,為減少其發(fā)病率,須積極控制尿路感染,術(shù)前術(shù)中發(fā)現(xiàn)輸尿管異常時,謹(jǐn)慎選擇雙J管粗細(xì)型號,術(shù)后恢復(fù)良好的情況下應(yīng)盡早拔管,減少留置時間。
