根治性膀胱切除聯(lián)合尿流改道術后尿道復發(fā)的危險因素及隨訪治療策略

目的:根治性膀胱切除聯(lián)合尿路改道是膀胱癌外科治療的常用手段,但同時具有尿道復發(fā)的風險。一系列研究報道了各種導致尿道復發(fā)的危險因素,但尚未有文獻對其預測風險價值進行統(tǒng)計分析。此外,隨訪周期和隨訪策略也與尿道復發(fā)率密切相關,目前也尚未系統(tǒng)性地建立完善的隨訪方案。因此,我們設計本次薈萃分析評估尿道復發(fā)的發(fā)生率,同時權衡不同危險因素對尿道復發(fā)的預測價值。此外,本次薈萃分析還總結了尿道復發(fā)患者的隨訪方案和治療方法,并分析了其與預后的關系。材料與方法:采用PubMed、EMBASE和MEDLINE檢索1971-2016年發(fā)表的研究。我們使用STATA軟件（版本12.0）來估計集中風險率,使用Cochran’sχ2-based Q和I2測試評估異質(zhì)性。結果:最終25個研究符合納入標準,共納入9498例患者。其中23人報告了參與者的性別,20人提供了患者的年齡,平均年齡為62.3歲（27歲至89歲）。在17項研究中描述了隨訪時間,平均為50.5個月（范圍從1月到254月）。原位癌患者的尿道復發(fā)率高于無原位癌患者。淺表性膀胱癌患者尿道復發(fā)率較高,膀胱內(nèi)型膀胱癌患者尿道復發(fā)率高于膀胱外型膀胱癌患者。與CS...
