【摘要】：目的探討經尿道前列腺等離子剜除術聯(lián)合恥骨上小切口治療高齡高危良性前列腺增生(BPH)合并膀胱結石的安全性及療效。方法采用經尿道前列腺等離子剜除術聯(lián)合恥骨上小切口治療42例高齡高危BPH合并膀胱結石患者,比較術前及術后臨床資料。結果該組患者均順利完成,無死亡病例,手術時間(63.90±16.60)min,術中出血量(73.90±21.10)ml,無需要輸血的病例,無1例發(fā)生前列腺電切綜合征,術后未發(fā)現(xiàn)結石殘留。手術前與手術后即刻生命特征及血K+、血清腦鈉肽(BNP)等內環(huán)境指標比較差異無統(tǒng)計學意義,術中術后均無嚴重內科并發(fā)癥發(fā)生。術后隨訪3個月,最大尿流率(Qmax)、殘余尿(PVR)、國際前列腺癥狀評分(IPSS)和生活質量評分(QOL)與術前比較,差異均有統(tǒng)計學意義(P0.05)。結論經尿道前列腺剜除術聯(lián)合小切口切開取石并取出剜除腺瘤的手術方式治療高齡高危BPH患者伴膀胱結石,安全性好,療效確切,特別適用于高齡高危的患者。
[Abstract]:Objective to evaluate the safety and efficacy of transurethral plasma enucleation of the prostate (TURP) combined with suprapubic small incision in the treatment of high risk benign prostatic hyperplasia (BPH) with bladder calculi. Methods Transurethral plasma enucleation of the prostate combined with suprapubic small incision was used to treat 42 elderly patients with high risk BPH complicated with bladder calculi. The clinical data before and after operation were compared. Results all the patients were successfully completed, there were no cases of death, the operative time was (63.90 鹵16.60) min, and the amount of blood loss was (73.90 鹵21.10) ml,. No residual stones were found after operation. There was no significant difference in life characteristics, blood K, serum brain natriuretic peptide (BNP) and so on before and immediately after operation, and no serious complications occurred during and after operation. Following up for 3 months, there were significant differences in maximum urinary flow rate (Qmax),) residual urine (PVR), international prostatic symptom score (IPSS) and quality of life score (QOL) compared with pre-operation (P0.05). Conclusion transurethral enucleation of prostate combined with small incision lithotomy and removal of adenoma is a safe and effective method for the treatment of high risk BPH patients with bladder calculi.
