【摘要】：目的探討不同途徑取石治療嵌頓性輸尿管上段結(jié)石的治療效果及對(duì)患者創(chuàng)傷程度的影響。方法 80例嵌頓性輸尿管上段結(jié)石患者分為經(jīng)皮腎輸尿管取石(MPCNL)的對(duì)照組和經(jīng)尿道輸尿管取石(URL)的觀察組各40例,比較兩種手術(shù)方式的治療效果及兩種手術(shù)方式對(duì)患者炎癥因子和應(yīng)激水平的影響。結(jié)果手術(shù)后1d觀察組患者丙二醛(MDA)、晚期氧化蛋白產(chǎn)物(AOPP)分別為(21.54±1.20)、(58.63±4.15)μmol/L,術(shù)后3、5dMDA、AOPP水平持續(xù)下降,均低于對(duì)照組患者;術(shù)后1d觀察組谷胱目肽過(guò)氧化物酶(GSH-PX)、超氧化物歧化酶(SOD)分別為(197.53±16.65)、(17.54±1.63)μg/mL,術(shù)后3、5d GSH-PX、SOD水平持續(xù)上升均高于對(duì)照組患者,術(shù)后1d觀察組腫瘤壞死因子α(TNF-α)、白細(xì)胞介素10(IL-10)水平分別為(33.34±5.17)、(56.54±6.63)μg/mL,術(shù)后3、5d持續(xù)下降,均明顯低于對(duì)照組患者,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論采用URL治療嵌頓性輸尿管上段結(jié)石對(duì)機(jī)體創(chuàng)傷較小,取石成功率高,值得臨床推廣運(yùn)用。
[Abstract]:Objective to investigate the effect of stone extraction in the treatment of incarcerated upper ureteral calculi and its influence on the degree of trauma. Methods Eighty patients with incarcerated upper ureteral calculi were divided into two groups: the control group (MPCNL) and the observation group (40 cases) with transurethral ureteral lithotripsy (URL). To compare the therapeutic effects of two surgical methods and their effects on inflammatory factors and stress levels. Results the level of malondialdehyde (MDA) (MDA), late oxidative protein (AOPP) was (21.54 鹵1.20), (58.63 鹵4.15) 渭 mol/L, in the observation group on the 1st day after operation, which was significantly lower than that in the control group. Glutathione peroxidase (GSH-PX) and superoxide dismutase (SOD) were (197.53 鹵16.65), (17.54 鹵1.63) 渭 g 路mL ~ (-1) 渭 g 路mL ~ (-1) in the observation group on the 1st day after operation. The GSH-PX,SOD level in the observation group was significantly higher than that in the control group on the 3rd and 5th day after operation. The levels of tumor necrosis factor 偽 (TNF- 偽) and interleukin-10 (IL-10) were (33.34 鹵5.17), (56.54 鹵6.63) 渭 g / mL in the observation group on the 1st day after operation. The difference was statistically significant (P0.05). Conclusion the treatment of incarcerated upper ureteral calculi with URL has less injury and higher success rate, so it is worth popularizing.
