【摘要】：目的探討Revolution CT全腎灌注成像去除動(dòng)脈期數(shù)據(jù)對腎透明細(xì)胞癌灌注參數(shù)值的影響。方法回顧性分析接受Revolution CT腎臟增強(qiáng)及灌注掃描、并經(jīng)手術(shù)病理證實(shí)為腎透明細(xì)胞癌的患者10例。采用Revolution CT機(jī)軸掃模式進(jìn)行灌注掃描,采用CT Perfusion 4D腎臟灌注軟件包進(jìn)灌注成像分析,對所有圖像均進(jìn)行2次后處理,第1次選擇所有的灌注數(shù)據(jù)為A組;第2次去除第9~15期數(shù)據(jù),共測量16期,為B組。選擇右側(cè)腎門水平腹主動(dòng)脈為輸入動(dòng)脈,獲得血流量(BF)圖、血容量(BV)圖、平均通過時(shí)間(MTT)圖及表面通透性(PS)圖。分別對比2組腫瘤與正常腎皮質(zhì)間各灌注參數(shù)的差異,并比較A組和B組間腫瘤、正常腎皮質(zhì)間各灌注參數(shù)值的差異。結(jié)果 A組和B組中腫瘤較正常腎皮質(zhì)的BF值及PS值均減低(P均0.05),BV及MTT值差異均無統(tǒng)計(jì)學(xué)意義(P均0.05)。2組病變側(cè)各灌注參數(shù)值差異均無統(tǒng)計(jì)學(xué)意義(P均0.05),2組正常腎皮質(zhì)的BF值差異有統(tǒng)計(jì)學(xué)意義(P=0.009),余灌注參數(shù)值差異亦無統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論灌注掃描時(shí)間為600s時(shí),去除動(dòng)脈期數(shù)據(jù)后測量的腎透明細(xì)胞癌各灌注參數(shù)值與包含動(dòng)脈期數(shù)據(jù)測量的結(jié)果無差異,去除動(dòng)脈期的數(shù)據(jù)可用于診斷腎透明細(xì)胞癌。
[Abstract]:Objective to investigate the effect of the removal of arterial phase data on the perfusion parameters of renal clear cell carcinoma (RCC) by revolution CT whole renal perfusion imaging. Methods Ten cases of renal clear cell carcinoma confirmed by surgery and pathology were analyzed retrospectively. The perfusion scan was performed using the axial scan mode of the Revolution CT, and the perfusion imaging analysis of renal perfusion software package was performed with CT Perfusion 4D. All the images were processed twice, and all the perfusion data were selected as group A for the first time, and the data of stage 915 was removed for the second time. A total of 16 phases were measured for group B. Blood flow (BF), blood volume (BV), mean transit time (MTT) and surface permeability (PS) were obtained. The perfusion parameters of tumor and normal renal cortex were compared between group A and group B, and the difference of perfusion parameters between normal renal cortex and tumor between group A and group B. Results the BF and PS values of tumor in group A and group B were lower than those in normal renal cortex (P 0.05). There was no significant difference in BV and MTT values between group A and group B (P 0.05). 2. There was no significant difference in perfusion parameters of pathological side between group A and group B (P 0.05). The BF value of cortex was significantly different (P0. 009), and there was no significant difference in residual perfusion parameters (P0. 05). Conclusion when the perfusion scan time is 600 s, the perfusion parameters of renal clear cell carcinoma after removing arterial phase data are not different from those of renal clear cell carcinoma including arterial phase data. The data removed from arterial phase can be used to diagnose renal clear cell carcinoma.
