【摘要】：目的：研究前列腺液Galectin-3濃度對(duì)診斷前列腺癌，以及鑒別進(jìn)展和轉(zhuǎn)移型前列腺癌的臨床應(yīng)用價(jià)值 方法：31名可疑為前列腺癌的中老年男性納入為實(shí)驗(yàn)對(duì)象，均行經(jīng)直腸B超引導(dǎo)下前列腺穿刺活檢。在前列腺穿刺之前行經(jīng)直腸前列腺按摩，取前列腺液。使用多克隆抗體ELISA法測(cè)前列腺液中Galectin-3濃度，使用人Galectin-3抗體免疫組化法對(duì)穿刺組織進(jìn)行染色，比較前列腺癌組和前列腺增生組的Galectin-3濃度及組織表達(dá)的差異。 結(jié)果：前列腺癌組和前列腺增生組相比，年齡與前列腺體積均無(wú)統(tǒng)計(jì)學(xué)差異。而PSA和PSAD有統(tǒng)計(jì)學(xué)差異，前列腺癌組大于前列腺增生組。前列腺液Galectin-3濃度前列腺癌組亦大于前列腺增生組（P0.001）。前列腺癌患者12人，其中有4人的骨核素掃描提示轉(zhuǎn)移性骨損害，其Galectin-3濃度顯著較其他前列腺癌者高。然而，Galectin-3和gleasson評(píng)分，PSA，，PSAD均無(wú)顯著相關(guān)性。Galectin-3的ROC曲線(xiàn)下面積最大，為0.921，其診斷前列腺癌的價(jià)值大于PSA和PSAD.當(dāng)Galectin-3濃度為4167ug/ml，敏感度為100%，特異度達(dá)到80%。免疫組化進(jìn)一步顯示，前列腺癌組織的Galectin-3表達(dá)量大于前列腺增生組，兩組有統(tǒng)計(jì)學(xué)差異。 結(jié)論：前列腺癌患者前列腺液的Galectin-3濃度顯著高于良性前列腺增生者，并且，發(fā)生骨轉(zhuǎn)移的前列腺癌Galectin-3濃度較未轉(zhuǎn)移的患者更高。前列腺液Galectin-3不僅可以作為診斷前列腺癌的有利指標(biāo)，也可以用來(lái)判斷轉(zhuǎn)移性前列腺癌的存在，從而指導(dǎo)治療和推斷預(yù)后。
[Abstract]:Objective: to study the clinical value of Galectin-3 concentration in prostatic fluid in the diagnosis of prostate cancer and the differential diagnosis and metastasis of prostate cancer.Methods 31 middle and aged men suspected to be prostate cancer were included in the study. All of them underwent transrectal ultrasound guided prostate biopsy.Before the prostate puncture, the prostate is massaged through the rectum and the prostatic fluid is taken.The concentration of Galectin-3 in prostatic fluid was measured by polyclonal antibody ELISA method, and the expression of Galectin-3 in prostate cancer group and prostatic hyperplasia group was compared by immunohistochemical staining with human Galectin-3 antibody.Results: there was no significant difference in age and prostate volume between prostate cancer group and prostatic hyperplasia group.But PSA and PSAD have statistical difference, prostate cancer group is bigger than prostatic hyperplasia group.The concentration of Galectin-3 in prostatic fluid in prostate cancer group was higher than that in prostatic hyperplasia group (P 0.001).Osteonuclide scanning of 4 of 12 patients with prostate cancer suggested metastatic bone damage, and their Galectin-3 levels were significantly higher than those of other prostate cancer patients.However, there was no significant correlation between Galectin-3 and gleasson score. The area under the ROC curve of Galectin-3 was the largest (0.921). The diagnostic value of Galectin-3 was higher than that of PSA and PSAD.When the concentration of Galectin-3 is 4167 ug.ml, the sensitivity is 100 and the specificity is 80.Immunohistochemical staining showed that the expression of Galectin-3 in prostate cancer was higher than that in benign prostatic hyperplasia (P < 0.05).Conclusion: the concentration of Galectin-3 in prostatic fluid of prostate cancer patients is significantly higher than that in patients with benign prostatic hyperplasia, and the concentration of Galectin-3 in prostate cancer with bone metastasis is higher than that in patients without metastasis.Prostatic fluid Galectin-3 can not only be used as a useful marker for the diagnosis of prostate cancer, but also can be used to judge the presence of metastatic prostate cancer, which can guide the treatment and predict the prognosis.
