【摘要】：目的：探討2型糖尿病患者踝臂指數(shù)（ABI）與尿微量白蛋白肌酐比（ACR）的相關(guān)性，，為防治糖尿病腎病提供新的臨床依據(jù)。 方法：共收集310例2型糖尿病患者，依據(jù)ABI測(cè)定結(jié)果分為ABI下降組（75例）及ABI正常組（235例），比較兩組病人間ACR指標(biāo)的差異，進(jìn)一步將ABI下降組的病人分為輕度下降組和中度下降組，比較兩組間ACR的差異，分析ABI與ACR的相關(guān)性。 結(jié)果：ABI下降組ACR異常率明顯高于ABI正常組，分別為68.00%、6.38%，組間差異有統(tǒng)計(jì)學(xué)意義（P0.05），兩組的年齡、病程、收縮壓、舒張壓、體重指數(shù)、總膽固醇、高密度脂蛋白膽固醇、低密度脂蛋白膽固醇、甘油三酯水平、白細(xì)胞計(jì)數(shù)、糖化血紅蛋白間差異均無(wú)統(tǒng)計(jì)學(xué)意義（P0.05），ABI中度下降組ACR異常率高于ABI輕度下降組，分別為84.00%、60%，組間差異有統(tǒng)計(jì)學(xué)意義（P0.05），相關(guān)分析提示ABI與ACR呈負(fù)相關(guān)（r=-0.412，P0.01）。 結(jié)論：ABI下降組ACR異常率較ABI正常組明顯升高，相關(guān)分析提示ABI與ACR顯著相關(guān)，提示ABI的變化可能是糖尿病腎病的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective: to investigate the correlation between ABI and urinary albumin creatinine ratio (ACRR) in type 2 diabetic patients, and to provide a new clinical basis for the prevention and treatment of diabetic nephropathy. Methods: a total of 310 patients with type 2 diabetes mellitus were divided into two groups according to the results of ABI measurement: 75 patients with decreased ABI and 235 patients with normal ABI. The difference of ACR indexes between the two groups was compared. The patients with decreased ABI were further divided into mild group and moderate group. The difference of ACR between the two groups was compared and the correlation between ABI and ACR was analyzed. Results the abnormal rate of ACR in the decreased group of ABI was significantly higher than that in the normal group (68.00 鹵6.38, P 0.05). The age, course of disease, systolic blood pressure, diastolic blood pressure, body mass index, total cholesterol, high density lipoprotein cholesterol in the two groups were significantly higher than those in the control group. There was no significant difference in low density lipoprotein cholesterol, triglyceride level, white blood cell count and glycosylated hemoglobin between the two groups. The abnormal rate of ACR in the moderate decrease group was higher than that in the mild ABI group. The difference between the two groups was statistically significant (P 0.05). The correlation analysis indicated that ABI and ACR were negatively correlated with ACR. Conclusion the abnormal rate of ACR in the descending group of ABI is significantly higher than that in the normal group of ABI. The correlation analysis indicates that ABI is significantly correlated with ACR, suggesting that the change of ABI may be an independent risk factor for diabetic nephropathy.
