【摘要】：目的研究后腹腔鏡與輸尿管鏡技術治療感染性輸尿管上段結石者后對其腎功能及C-反應蛋白(CRP)、白介素-6(IL-6)、白細胞計數(shù)(WBC)的影響,以期為感染性輸尿管上段結石的治療提供數(shù)據(jù)依據(jù)。方法收集2014年1月-2015年12月醫(yī)院經(jīng)輸尿管內(nèi)碎石技術(URSL)及腹腔鏡輸尿管切開取石術(RPLU)治療感染性輸尿管上段結石患者的臨床資料;采用URSL治療124例,為URSL組,采用RPLU治療94例,為RPLU組,對比分析兩組患者的臨床資料、腎功能指標、炎癥指標、術后并發(fā)癥及術后結石清除情況。結果兩組患者手術前BUN、SCr、NGAL及CRP、IL-6、WBC對比,差異無統(tǒng)計學意義,URSL組患者術后NGAL水平高于同組治療前及RPLU組同期(P0.05),RPLU組患者術后1dCRP、IL-6水平高于同組術前及URSL組同期(P0.05),術后7dCRP、IL-6及WBC水平低于同組術前(P0.05);兩組術后并發(fā)癥比較,差異無統(tǒng)計學意義,兩組30d治療效果均為100.00%,差異無統(tǒng)計學意義。結論后腹腔鏡與輸尿管鏡技術均可有效治療感染性輸尿管上段結石,兩種術式各有利弊,應結合臨床選擇術式。
[Abstract]:Objective to study the effects of retroperitoneal laparoscopy and ureteroscopy on renal function, C-reactive protein (CRP), interleukin-6 (IL-6) and white blood cell count (WBC) in patients with upper ureteral calculi. Methods from January 2014 to December 2015, hospital transureteral lithotripsy (URSL) and laparoscopic ureterolithotomy (RPLU) were collected for the treatment of infectious upper ureteral calculi. Clinical data of stone patients; 124 cases were treated with URSL, URSL group, 94 cases were treated with RPLU, and 94 cases were treated with RPLU. The clinical data, renal function index and inflammation index of the two groups were compared and analyzed. Results before operation, the patients in the two groups were compared with BUN-SCrNGAL and CRPIL-6 WBC. There was no significant difference in postoperative NGAL levels between the two groups. The level of NGAL was significantly higher in Url group than that in the same group before treatment and 1 day after operation in RPLU group. The level of IL-6 and WBC in URSL group was higher than that in the same group and URSL group at 1 day after operation, and the levels of IL-6 and WBC in URSL group were lower than those in the same group at 7 days after operation (P 0.05), and the postoperative complications were compared between the two groups. There was no significant difference between the two groups in 30 days treatment, the difference was not statistically significant. Conclusion retroperitoneal laparoscopy and ureteroscopy are effective in the treatment of infectious upper ureteral calculi, both of which have advantages and disadvantages, and should be combined with clinical choice.
