【摘要】:背景:臨床上重建長(zhǎng)段輸尿管缺損的方法存在并發(fā)癥多且創(chuàng)傷大等缺點(diǎn),因此,需要尋找其他的修復(fù)方式。目的:探討組織工程管狀移植物修復(fù)長(zhǎng)段輸尿管缺損的可行性。方法:將兔骨髓間充質(zhì)干細(xì)胞和膀胱平滑肌細(xì)胞種植到兔膀胱脫細(xì)胞基質(zhì)的兩面,培養(yǎng)7 d后制作成4 cm長(zhǎng)的組織工程管狀移植物,僅將膀胱平滑肌細(xì)胞種植到兔膀胱脫細(xì)胞基質(zhì)構(gòu)建組織工程管狀移植物作為對(duì)照;新西蘭兔25只,隨機(jī)分為實(shí)驗(yàn)組20只和對(duì)照組5只,將2種組織工程管狀移植物移植到兔體內(nèi)修復(fù)輸尿管缺損,修復(fù)同時(shí)利用網(wǎng)膜包裹,分別于術(shù)后2,4,8周取實(shí)驗(yàn)組標(biāo)本做蘇木精-伊紅染色及免疫組化檢測(cè)。結(jié)果與結(jié)論:組織學(xué)檢查發(fā)現(xiàn)實(shí)驗(yàn)組尿路上皮層再生呈連續(xù)性,術(shù)后8周可見(jiàn)組織工程管狀移植物管腔內(nèi)覆蓋多層尿路上皮結(jié)構(gòu)及規(guī)則的平滑肌束,免疫組化可見(jiàn)uroplakinⅢa及CK AE1/AE3陽(yáng)性,證實(shí)了組織工程管狀移植物組織內(nèi)有成熟上皮細(xì)胞覆蓋,且上皮細(xì)胞之間有連接功能。對(duì)照組5只動(dòng)物術(shù)后拔除輸尿管支架管2周內(nèi)均死亡,尸檢見(jiàn)流出道瘢痕形成,組織工程管狀移植物內(nèi)腔閉鎖,同側(cè)腎臟重度積水,未作組織學(xué)觀察。以上結(jié)果表明將骨髓間充質(zhì)干細(xì)胞和膀胱平滑肌細(xì)胞種植于兔膀胱脫細(xì)胞基質(zhì)上構(gòu)建組織工程管狀移植物修復(fù)長(zhǎng)段輸尿管缺損是可行的,組織工程管狀移植物內(nèi)腔有成熟上皮覆蓋,骨髓間充質(zhì)干細(xì)胞能促進(jìn)尿路上皮再生。
[Abstract]:Background: the method of reconstruction of long ureteral defect in clinic has the disadvantages of many complications and great trauma. Objective: to explore the feasibility of repairing long ureteral defects with tissue engineered tubular grafts. Rabbit bone marrow mesenchymal stem cells and bladder smooth muscle cells were implanted on both sides of acellular matrix of rabbit bladder. After 7 days of culture, 4 cm long tissue engineered tubular grafts were prepared, and only bladder smooth muscle cells were implanted into acellular matrix of rabbit bladder to construct tissue-engineered tubular grafts as control. Twenty-five New Zealand rabbits were randomly divided into two groups: experimental group (n = 20) and control group (n = 5). Two kinds of tissue engineered tubular grafts were transplanted into rabbits to repair ureteral defects. The specimens of the experimental group were collected for hematoxylin-eosin staining and immunohistochemistry. Results and conclusion: histological examination showed that there was continuity in the regeneration of the upper cortex of the urinary tract in the experimental group. Eight weeks after operation, tissue engineering tubular grafts were found to be covered with multiple layers of urinary epithelial structure and regular smooth muscle bundles.
