【摘要】：目的分析單中心腎嫌色細(xì)胞癌的臨床特征,探討與腎嫌色細(xì)胞癌預(yù)后相關(guān)的臨床指標(biāo)。方法通過回顧性分析2005年1月—2015年5月在上海長海醫(yī)院住院手術(shù),且經(jīng)術(shù)后病理學(xué)檢查證實(shí)為腎嫌色細(xì)胞癌的113例患者的臨床、病理和隨訪資料。研究終點(diǎn)為總生存期,生存分析數(shù)據(jù)采用Kaplan-Meier法進(jìn)行計算,并用Log-rank法對生存率進(jìn)行分析比較。結(jié)果腎嫌色細(xì)胞癌占同期所有腎細(xì)胞癌的4.8%(113/2 355),患者中位年齡為50.96歲,男女比例為1.01∶1,80例(70.8%)無癥狀。腎嫌色細(xì)胞癌的1、3、5年生存率分別為98.2%(108/110)、96.4%(106/110)、95.5%(105/110)。本組中Ⅰ型腎嫌色細(xì)胞癌107例,Ⅱ型腎嫌色細(xì)胞癌6例。腎嫌色細(xì)胞癌單因素Kaplan-Meier預(yù)后分析顯示,Ⅱ型腎嫌色細(xì)胞癌的預(yù)后有好于Ⅰ型的趨勢,但差異無統(tǒng)計學(xué)意義(P=0.515);不同癥狀、不同T分期和美國癌癥聯(lián)合委員會(AJCC)分期的腎嫌色細(xì)胞癌患者預(yù)后的差異均有統(tǒng)計學(xué)意義(P值均0.05)。結(jié)論腎嫌色細(xì)胞癌是預(yù)后較好的腎癌亞型之一,其中Ⅱ型預(yù)后相對更好。患者的癥狀、腫瘤的大小和AJCC分期可以作為判斷腎嫌色細(xì)胞癌預(yù)后的重要因素。
[Abstract]:Objective to analyze the clinical characteristics of single center of chromophobe cell renal carcinoma and to investigate the clinical indicators related with prognosis of chromophobe renal cell carcinoma. Methods a retrospective analysis from January 2005 to May 2015 in Changhai Hospital of Shanghai during surgery, and the postoperative pathological examination confirmed renal chromophobe cell carcinoma 113 cases of patients with clinical, pathological and follow-up data. The end point was the total survival time, the survival analysis data were calculated by Kaplan-Meier method, and the survival rate was compared with the Log-rank method. 4.8% of all renal cell carcinoma results of chromophobe renal cell carcinoma (113/2 355) for the same period, the median age was 50.96 years old, male and female ratio of 1.01 1,80 cases (70.8%) without symptoms. Chromophobe renal cell carcinoma, 1, 3, 5 year survival rates were 98.2% (108/110), 96.4% (106/110), 95.5% (105/110). 107 cases of type I in this group of chromophobe renal cell carcinoma, 6 cases of type II renal chromophobe cell carcinoma. Chromophobe renal cell carcinoma Kaplan-Meier univariate prognostic analysis showed that there is good to the type of prognosis of type II chromophobe renal cell carcinoma, but the difference was not statistically significant (P=0.515); different symptoms and T staging and the American Joint Committee on cancer (AJCC) chromophobe renal cell carcinoma prognosis staging were statistically significant (the P value was 0.05). Conclusion chromophobe renal cell carcinoma is one of the better prognosis of renal cell carcinoma subtype, type II, the prognosis is relatively better. The patient's symptoms, tumor size and AJCC stage can be used as an important factor to determine the prognosis of chromophobe renal cell carcinoma.
