【摘要】：目的評價右美托咪啶對口腔頜面腫瘤根治術(shù)患者經(jīng)鼻清醒插管應(yīng)激反應(yīng)的影響。方法選取擇期行口腔頜面腫瘤根治術(shù)患者40例,ASAⅠ或Ⅱ級,年齡20~62歲,性別不限,體質(zhì)指數(shù)25。按隨機(jī)數(shù)字表法分為兩組(n=20):右美托咪啶組(D組)和咪達(dá)唑侖組(M組)。記錄平均動脈壓(MAP)、心率(HR)、血氧飽和度(Sp O2)變化,記錄呼吸抑制發(fā)生率、患者對插管過程的耐受程度、患者的配合程度和術(shù)后對插管過程的不良記憶情況。結(jié)果兩組T1的HR比T0時較均升高,差異具有統(tǒng)計學(xué)意義(P0.05)。M組T1-2時的MAP和HR及T3時HR比T0時升高,差異具有統(tǒng)計學(xué)意義(P0.05)。與M組比較,D組T1-2時的MAP和HR,T3時HR降低,差異具有統(tǒng)計學(xué)意義(P0.05)。D組呼吸抑制發(fā)生率較M組降低,氣管插管舒適度和配合程度評分較M組降低(P0.05)。結(jié)論右美托咪啶可以減輕口腔頜面腫瘤根治術(shù)患者經(jīng)鼻清醒氣管插管的應(yīng)激反應(yīng),提高患者舒適度,減少呼吸抑制發(fā)生率,維持血流動力學(xué)穩(wěn)定,保證了插管的安全性。
[Abstract]:Objective to evaluate the effect of dexmetidine on stress response in patients undergoing radical resection of oral and maxillofacial tumors. Methods A total of 40 patients with oral and maxillofacial neoplasms underwent radical resection of oral and maxillofacial tumors, aged 20 to 62 years, aged 20 to 62 years, with body mass index (BMI) of 25. According to the method of random number table, they were divided into two groups: dexmetomidine group (group D) and midazolam group (group M). The changes of mean arterial pressure (map), heart rate (HRN) and oxygen saturation (Sp / O _ 2) were recorded. The incidence of respiratory inhibition, the tolerance of patients to intubation, the degree of cooperation of patients and the bad memory during intubation were recorded. Results the HR of T1 was higher than that of T0, the difference was statistically significant (P 0.05). The MAP and HR of group M at T1-2 and HR at T3 were higher than those of T0. The difference was statistically significant (P 0.05). Compared with group M, the MAP and HR at T1-2 and HRT 3 in group D were significantly lower than those in group M, and the incidence of respiratory inhibition in group D was lower than that in group M, and the score of tracheal intubation comfort and cooperation degree was lower than that in group M (P 0.05). Conclusion dexmetidine can reduce the stress response of tracheal intubation in patients with oral and maxillofacial neoplasms, increase the comfort of patients, reduce the incidence of respiratory inhibition, maintain hemodynamic stability, and ensure the safety of intubation.
