低氣腹壓腹腔鏡治療IVF-ET術(shù)后早孕合并宮外孕效果觀察

【摘要】：目的觀察低氣腹壓腹腔鏡治療體外受精—胚胎移植(IVF-ET)術(shù)后早孕合并宮外孕的效果。方法IVF-ET術(shù)后早孕合并宮外孕患者42例,按照患者意愿自主選擇手術(shù)方式,其中選擇低氣腹壓腹腔鏡治療22例(腹腔鏡組)、開腹探查20例(開腹組),比較兩組術(shù)中出血量、手術(shù)時(shí)間、抗生素使用時(shí)間、住院時(shí)間及術(shù)后肛門排氣時(shí)間、疼痛評(píng)分、妊娠過程和結(jié)局。結(jié)果腹腔鏡組術(shù)中出血量(22.1±6.6)m L、手術(shù)時(shí)間(29.3±9.2)min、抗生素使用時(shí)間(2.1±0.7)d、住院時(shí)間(4.1±0.9)d、術(shù)后肛門排氣時(shí)間(18.4±6.7)h、術(shù)后疼痛評(píng)分(1.5±0.8)分,開腹組分別為(51.4±8.9)m L、(42.5±9.3)min、(3.2±0.6)d、(5.5±0.4)d、(42.6±8.9)h、(4.6±1.6)分,兩組比較,P均0.05。腹腔鏡組患者均足月分娩;新生兒1分鐘Apgar評(píng)分7分1例,8~10分21例;平均出生體質(zhì)量3 250 g;無一例新生兒畸形。開腹組患者中自然流產(chǎn)1例,其余19例足月分娩,孕周平均38周;新生兒1分鐘Apgar評(píng)分8~10分19例;平均出生體質(zhì)量3 200 g;無一例新生兒畸形。結(jié)論低氣腹壓腹腔鏡治療IVF-ET術(shù)后早孕合并宮外孕效果較好,且較開腹手術(shù)者術(shù)中出血量少,手術(shù)時(shí)間、抗生素使用時(shí)間、住院時(shí)間、術(shù)后肛門排氣時(shí)間短,術(shù)后疼痛評(píng)分低。
[Abstract]:Objective to observe the effect of laparoscopy with low pneumoperitoneum in the treatment of early pregnancy complicated with ectopic pregnancy after in vitro fertilisation and embryo transfer (IVF-ET). Methods 42 patients with early pregnancy complicated with ectopic pregnancy after IVF-ET were selected according to their wishes. 22 patients were treated by laparoscopy with low pneumoperitoneum pressure (laparoscopy group) and 20 patients with laparotomy (laparotomy group). The amount of intraoperative bleeding, operation time, antibiotic use time, hospitalization time, postoperative anal exhaust time, pain score, pregnancy process and outcome were compared between the two groups. Results in the laparoscopic group, the intraoperative bleeding volume was (22.1 鹵6.6) m L, the operation time was (29.3 鹵9.2) min, the hospitalization time was (4.1 鹵0.9) d, the postoperative anal exsufflation time was (18.4 鹵6.7) h, and the postoperative pain score was (1.5 鹵0.8) d.
