【摘要】：目的:分析精子因素對卵胞質內單精子注射(intracytoplasmic sperm injection,ICSI)臨床結局的影響。方法:回顧性分析不同來源和參數精液行ICSI助孕的臨床資料,A組為因精卵結合障礙重復周期行ICSI授精者228周期,B組少弱精子組790周期,C組為附睪穿刺取精(percutaneous epididymal sperm aspiration,PESA)/睪丸穿刺取精(testicular sperm aspiration,TESA)組225周期,比較各組患者行ICSI助孕的胚胎質量及臨床結局。結果:3組患者的正常受精率和卵裂率均無統計學差異,A組患者可利用胚胎率明顯低于B組,A組患者的優(yōu)質胚胎率明顯低于B、C 2組;新鮮移植周期中A組患者胚胎種植率明顯低于B、C2組,臨床妊娠率、多胎率、早期流產率、早產率、活產率及新生兒男女性別比例均無顯著差異(P0.05),A組患者新生兒體質量高于B、C 2組,且B組高于C組;復蘇移植周期A組患者臨床妊娠率和胚胎種植率明顯低于B、C 2組,且C組大于B組,C組活產率明顯大于A、B 2組(P0.05),其他臨床指標無統計學差異(P0.05)。結論:精子的參數及來源不影響ICSI治療的受精、卵裂及早期妊娠;ICSI能夠改善受精障礙組的受精情況(A組),但胚胎的質量和發(fā)育潛能較低;PESA/TESA組有相對較高的臨床妊娠率和種植率。
[Abstract]:Objective: to analyze the effect of sperm factors on the clinical outcome of intracytoplasmic sperm injection of (intracytoplasmic sperm injection,ICSI. Methods: the clinical data of ICSI assisted pregnancy with semen from different sources and parameters were analyzed retrospectively. the embryo quality and clinical outcome of ICSI assisted pregnancy in group A were compared with those in group B (oligozoospermia group), group B (oligozoospermia group) and group C (epididymal puncture / testicular sperm extraction (testicular sperm aspiration,TESA). Results: there was no significant difference in normal fertilizing rate and cleavage rate among the three groups. The available embryo rate in group A was significantly lower than that in group B, and the rate of high quality embryo in group A was significantly lower than that in group B. In the fresh transplantation cycle, the embryo implantation rate of group A was significantly lower than that of group B. there were no significant differences in clinical pregnancy rate, multiple births rate, early abortion rate, premature delivery rate, live delivery rate and sex ratio of newborn between male and female in group C (P 0.05). The body mass of neonates in group), A was higher than that in group B, and group B was higher than group C. The clinical pregnancy rate and embryo implantation rate in group A were significantly lower than those in group B, group C was higher than group B, and the live birth rate in group C was significantly higher than that in group A. there was no significant difference in other clinical indexes between group A and group B (P 0.05). Conclusion: the parameters and sources of spermatozoa do not affect the fertilization, cleavage and early pregnancy treated with ICSI. ICSI can improve the fertilization in group A (group A), but the quality and developmental potential of embryos are low, and the clinical pregnancy rate and implantation rate in PESA/TESA group are relatively high.
