【摘要】：目的比較經(jīng)陰道超聲與經(jīng)腹部超聲檢查對(duì)宮角妊娠的診斷價(jià)值。方法將我院婦產(chǎn)科2015年5月至2016年5月期間收治的80例疑為宮角妊娠的患者作為研究對(duì)象,所有患者均先行經(jīng)腹部超聲檢查,再行經(jīng)陰道超聲檢查,以病理診斷結(jié)果為金標(biāo)準(zhǔn),比較兩種檢查方式對(duì)宮角妊娠的診斷準(zhǔn)確率,總結(jié)宮角妊娠的超聲特征。結(jié)果經(jīng)腹部超聲診斷對(duì)宮角妊娠的診斷敏感性為61.33%、特異性為20.00%、陽(yáng)性預(yù)測(cè)值為92.00%、陰性預(yù)測(cè)值為3.33%、準(zhǔn)確率為58.75%;經(jīng)陰道超聲對(duì)宮角妊娠的診斷敏感性為82.67%、特異性為80.00%、陽(yáng)性預(yù)測(cè)值為98.41%、陰性預(yù)測(cè)值為23.53%、準(zhǔn)確率為82.50%。經(jīng)陰道超聲檢查對(duì)宮角妊娠的診斷準(zhǔn)確性、敏感性、特異性、陰性預(yù)測(cè)值均明顯高于經(jīng)腹部超聲檢查,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。經(jīng)腹部超聲與經(jīng)陰道超聲聯(lián)合診斷宮角妊娠的準(zhǔn)確性、敏感性、特異性、陽(yáng)性預(yù)測(cè)值、陰性預(yù)測(cè)值分別為90.00%、90.67%、80.00%、98.55%、36.36%。結(jié)論與腹部超聲檢查比較,經(jīng)陰道超聲檢查對(duì)宮角妊娠的診斷準(zhǔn)確率更高,并且二者聯(lián)用能夠有效提高宮角妊娠的診斷敏感性與準(zhǔn)確性,能夠?yàn)榕R床早期診斷、處理宮角妊娠提供有效依據(jù)。
[Abstract]:Objective To compare the value of transvaginal and transabdominal ultrasonography in the diagnosis of uterine horn pregnancy. Methods From May 2015 to May 2016,80 cases of suspected uterine horn pregnancy were treated as the subject of the study. All the patients were examined by transvaginal ultrasound, and the result of pathological diagnosis was the gold standard. To compare the diagnostic accuracy of the two methods to the pregnancy of the uterine horn, and to sum up the ultrasonic characteristics of the uterine horn pregnancy. Results The diagnostic sensitivity of abdominal ultrasound was 61.33%, the specificity was 20.00%, the positive predictive value was 92.00%, the negative predictive value was 3.33%, the accuracy rate was 58.75%, and the diagnostic sensitivity of transvaginal ultrasound to the uterine horn pregnancy was 82.67% and the specificity was 80.00%. The positive predictive value was 98.41%, the negative predictive value was 23.53%, and the accuracy rate was 82.50%. The accuracy, sensitivity, specificity and negative predictive value of transvaginal ultrasonography in the diagnosis of uterine horn pregnancy were significantly higher than that of transabdominal ultrasonography (P0.05). The accuracy, sensitivity, specificity, positive predictive value and negative predictive value of abdominal ultrasound combined with transvaginal ultrasound were 90.00%, 90.67%, 80.00%, 98.55%, 36.36%, respectively. Conclusion Compared with abdominal ultrasonography, transvaginal ultrasonography is more accurate in the diagnosis of uterine horn pregnancy, and the combination of the two methods can effectively improve the diagnostic sensitivity and accuracy of the uterine horn pregnancy, and can provide an effective basis for the early diagnosis and treatment of the uterine horn pregnancy.
