【摘要】：目的探討孕前體重指數(shù)及孕期體重增加與巨大兒的發(fā)生率之間的關(guān)系。方法回顧性的分析首都醫(yī)科大學(xué)附屬北京婦產(chǎn)醫(yī)院2014年1月至2014年3月住院分娩的單胎、妊娠滿37~41周、無并發(fā)癥及慢性疾病初產(chǎn)婦的臨床資料共1 085例。根據(jù)世界衛(wèi)生組織的體重標準,所有病例按產(chǎn)婦孕前體重指數(shù)(BMI)分為低體重組BMI18.5kg/m2、標準體重組BMI 18.5~24.9kg/m2、超重組BMI 25~29.9kg/m2、肥胖組BMI≥30kg/m2四個組;孕期體重增加分為低于IOM標準,符合IOM標準,超出IOM標準三組。采用χ2檢驗法比較各組、各變量間差異的統(tǒng)計學(xué)意義。結(jié)果孕前低體重指數(shù)孕婦占17.42%,孕前標準體重指數(shù)孕婦占72.72%。超重及肥胖孕婦占9.9%,其中1.4%達到肥胖標準。超重及肥胖組巨大兒的發(fā)生率隨孕期體重增加而升高(P0.01)。孕期體重增加符合IOM標準時,仍有高度統(tǒng)計學(xué)意義(P0.01)。隨孕前BMI的增加,巨大兒的發(fā)生率大幅度的增加(P0.01)。結(jié)論隨著孕前BMI及孕期體重增加的增加巨大兒的發(fā)生率增加。提示加強對高危人群的孕前和孕期體重管理有助于預(yù)防巨大兒的發(fā)生、改善母嬰預(yù)后。
