HDAC4在宮頸癌組織中的表達(dá)及其臨床意義

【摘要】:目的: 檢測(cè)HDAC4在宮頸癌中的蛋白表達(dá)狀況,分析其蛋白表達(dá)水平與宮頸癌臨床病理參數(shù)之間的關(guān)系。 方法: 收集中南大學(xué)湘雅二醫(yī)院婦科2009年11月至2013年11月宮頸癌手術(shù)切除標(biāo)本280例。按照臨床分期及術(shù)后病檢結(jié)果分為:無(wú)淋巴轉(zhuǎn)移宮頸癌組和淋巴轉(zhuǎn)移或脈管浸潤(rùn)宮頸癌組。以宮頸高級(jí)別CIN、正常宮頸組織為實(shí)驗(yàn)對(duì)照。免疫組織化學(xué)S-P法檢測(cè)HDAC4蛋白在宮頸癌、高級(jí)別CIN和正常宮頸組織中的表達(dá),比較HDAC4在正常宮頸組織、高級(jí)別CIN、宮頸癌中的表達(dá)差異,并分析HDAC4蛋白表達(dá)與宮頸癌臨床病理參數(shù)的相關(guān)性。 結(jié)果: HDAC4蛋白陽(yáng)性表達(dá)主要位于癌細(xì)胞胞核,少數(shù)癌細(xì)胞胞漿有部分弱陽(yáng)性染色。HDAC4在正常宮頸、高級(jí)別CIN和宮頸癌組織中的陽(yáng)性表達(dá)率分別為20%(4/20)、35%(7/20)和88.6%(248/280),宮頸癌組織中HDAC4蛋白陽(yáng)性表達(dá)率顯著高于高級(jí)別CIN和正常宮頸組織(P=0.000);HDAC4在宮頸癌無(wú)淋巴轉(zhuǎn)移組的陽(yáng)性表達(dá)率(87.2%,158/181)與宮頸癌有淋巴轉(zhuǎn)移或脈管浸潤(rùn)組中的陽(yáng)性表達(dá)率(90.9%,90/99)比較無(wú)顯著性差異(P=0.241);但淋巴轉(zhuǎn)移或脈管浸潤(rùn)組宮頸癌組織中的HDAC4強(qiáng)陽(yáng)性表達(dá)率(47.4%,47/99)顯著高于宮頸癌無(wú)淋巴轉(zhuǎn)移組(35.9%,65/181)(P=0.039); HDAC4在宮頸癌Ⅰ期和Ⅱ期中總陽(yáng)性表達(dá)率、和強(qiáng)陽(yáng)性表達(dá)率分別為89.6%(138/154)、35.7%(55/154)和87.3%(116/126)、45.2%(57/126),經(jīng)卡方檢驗(yàn)比較,無(wú)顯著性差異(P=0.338)。HDAC4在宮頸腺癌、宮頸鱗癌中的陽(yáng)性表達(dá)率和強(qiáng)陽(yáng)性表達(dá)率分別為:80%(16/20)、25%(5/20)和88.9%(225/253)、40.3%(102/253)。經(jīng)卡方檢驗(yàn)比較,差異無(wú)顯著性(P=0.194)。HDAC4在宮頸癌病理分級(jí)為高、中、低分化中的陽(yáng)性表達(dá)率和強(qiáng)陽(yáng)性表達(dá)率分別為:84%(36/44)、40.9%(18/44),85.7%(98/116)和44.8%(52/116)83.6%(92/110)、37.3%(41/110),經(jīng)卡方檢驗(yàn)比較,差異無(wú)顯著性(P=0.963)。結(jié)果提示,HDAC4蛋白陽(yáng)性表達(dá)率從正常宮頸組織、高級(jí)別CIN到宮頸癌逐漸升高,宮頸癌組織中高HDAC4蛋白,且其過(guò)度表達(dá)與宮頸癌的脈管侵潤(rùn)和淋巴轉(zhuǎn)移相關(guān),但與宮頸癌的臨床分期、病理分級(jí)均無(wú)明顯的相關(guān)性。 結(jié)論: 正常宮頸組織中低表達(dá)HDAC4蛋白,宮頸癌組織中存在HDAC4蛋白高表達(dá),且其過(guò)度表達(dá)與宮頸癌淋巴轉(zhuǎn)移相關(guān),提示HDAC4可能與宮頸癌的發(fā)生、發(fā)展和淋巴轉(zhuǎn)移相關(guān),可作為宮頸癌診斷和其淋巴轉(zhuǎn)移的候選分子靶標(biāo)。 圖6張,表3個(gè),參考文獻(xiàn)37篇
