非醫(yī)學(xué)專業(yè)大學(xué)生對人乳頭瘤病毒及其疫苗認(rèn)知現(xiàn)況調(diào)查分析

【摘要】:目的:通過對非醫(yī)學(xué)專業(yè)類大學(xué)生群體的問卷調(diào)查,研究他們對于HPV、HPV相關(guān)疾病和疫苗相關(guān)知識的認(rèn)知水平以及他們對于HPV疫苗的接種意愿。為HPV疫苗應(yīng)用的決策者提供依據(jù),找出國內(nèi)推廣HPV疫苗存在的潛在障礙,為制定我國HPV預(yù)防性疫苗的接種政策提出科學(xué)建議。 方法:本研究基于《中國大學(xué)生對人乳頭瘤病毒及HPV預(yù)防性疫苗認(rèn)知的多中心研究》設(shè)計要求,采用橫斷面流行病學(xué)調(diào)查方法,采取分層整群抽樣方法在大連市選取兩所大學(xué)的學(xué)生(一所一類本科大學(xué)、一所三類本科學(xué)院的18-22歲非醫(yī)學(xué)專業(yè)的本科學(xué)生)為調(diào)查對象。應(yīng)用統(tǒng)一設(shè)計的調(diào)查表進(jìn)行匿名調(diào)查,調(diào)查問卷由被調(diào)查者在知情同意的情況下獨立填寫。調(diào)查數(shù)據(jù)由經(jīng)過培訓(xùn)的研究者運(yùn)用EpiData數(shù)據(jù)庫進(jìn)行雙份錄入和管理。采用SPSS13.0對調(diào)查數(shù)據(jù)進(jìn)行統(tǒng)計描述,采用χ2檢驗,檢驗水平為0.05。 結(jié)果:非醫(yī)學(xué)專業(yè)大學(xué)生對HPV知曉率偏低,總體知曉率僅為16.7%。按年級、專業(yè)及學(xué)籍類別分組分析,得到HPV知曉率情況。一類本科大學(xué)生的知曉率為16.5%,三類本科大學(xué)生的知曉率為17.3%,差異沒有統(tǒng)計學(xué)意義(P0.05)。一本大學(xué)生中:低年級學(xué)生的知曉率為13.0%,高年級學(xué)生的知曉率為21.0%,差異不顯著(P0.05);文科學(xué)生和理科學(xué)生的知曉率分別為27.5%和7.8%,差異顯著(P0.05);文科生中男生HPV知曉率是37.1%,女生知曉率是17.3%,差異無統(tǒng)計學(xué)意義(P0.05);理科生中男生HPV知曉率為8.7%,女生為6.6%,差異未見有統(tǒng)計學(xué)意義(P0.05);文科低年級學(xué)生中聽說過HPV的占30.0%,高年級學(xué)生占25.4%,無統(tǒng)計學(xué)意義(P0.05);理科低年級學(xué)生中認(rèn)知率僅2.2%,而高年級則為16.7%,P0.05,有顯著差異。三類本科大學(xué)生總體HPV知曉率為17.3%。低年級學(xué)生為15.9%,高年級學(xué)生為21.4%;三本男生的知曉率為18.8%,女生知曉率為13.3%,差異均無統(tǒng)計學(xué)意義。有過性行為的大學(xué)生知曉率為30.8%,明顯高于無性生活史的大學(xué)生的認(rèn)知率15.1%,P0.05,差異有統(tǒng)計學(xué)意義。大學(xué)生群體對HPV相關(guān)疾病知識的認(rèn)知均不高。 被調(diào)查大學(xué)生HPV疫苗整體知曉率僅為12.8%。一本知曉率15.4%高于三本知曉率6.4%,P0.05,差異顯著;一本文科學(xué)生知曉率為27.5%,遠(yuǎn)遠(yuǎn)高于理科生的知曉率5.9%,,P=0.00;在文科學(xué)生中男女生知曉率分別為29%,25.9%;高年級文科生的知曉率為35.6%,低年級為19.7%,差異不明顯。低年級理科生知曉率(2.2%)遠(yuǎn)低于高年級理科生的知曉率(11.7%),二者差異有統(tǒng)計學(xué)意義(P=0.029)。三本學(xué)生對HPV疫苗知曉率均偏低,且低、高年級間及性別間的差異均沒有統(tǒng)計學(xué)意義(P0.05)。 聽說過HPV疫苗的大學(xué)生里只有61.2%了解該疫苗預(yù)防何種疾病。有68.4%的大學(xué)生愿意接種疫苗。知曉HPV的大學(xué)生中有76.9%表示愿意接種疫苗,不知曉HPV的大學(xué)生有66.5%愿意接種疫苗,OR值為3.84,其95%可信區(qū)間為1.83~7.04。大學(xué)生對適合接種HPV疫苗的機(jī)構(gòu)選擇,主要集中在疾病預(yù)防控制中心(56.9%)和綜合醫(yī)院(11.7%)。 疫苗接種的最佳人群選項,只有9.9%的大學(xué)生選擇了尚無初次性行為者作為疫苗接種的最佳人群。26.6%的大學(xué)生認(rèn)為接種HPV預(yù)防性疫苗的最佳年齡段是16~18歲,19.6%的人認(rèn)為19~21歲為接種的最佳年齡,還有28.5%的人不知道最佳的接種年齡應(yīng)是多大。不愿意接種疫苗的主要原因除擔(dān)心疫苗的安全性外(占92.1%),還擔(dān)心目前疫苗沒有大范圍推廣使用(占47.2%)。無論是國產(chǎn)疫苗或進(jìn)口疫苗,大學(xué)生能承擔(dān)的最高價格均不超過300元,并且認(rèn)為疫苗上市政府應(yīng)該補(bǔ)貼72.5%以上的費用。 結(jié)論:1、大學(xué)生群體對HPV和HPV疫苗的認(rèn)知程度較低。大部分人不清楚HPV疫苗最佳接種年齡及最有效接種人群。2、大學(xué)生群體對HPV疫苗接種意愿較高。愿意接種的原因為接種疫苗能降低感染HPV的風(fēng)險,可使自己及伴侶從中獲益;不愿意接種HPV疫苗的主要原因有疫苗的價格過高、目前疫苗還沒有大范圍推廣使用及擔(dān)心疫苗的安全性和有效性等。3、大學(xué)生群體對疫苗的支付能力有限,能承擔(dān)的最高價格不超過300元,并且希望政府在疫苗價格方面給予相應(yīng)補(bǔ)貼。
