【摘要】：目的:探索米索前列醇不同時(shí)間給藥防治二次剖宮產(chǎn)患者產(chǎn)后出血的臨床效果。方法:將入選的126例二次剖宮產(chǎn)孕婦按照入院先后編號(hào),隨機(jī)分為三組:縮宮素組(A組,42例),術(shù)中、術(shù)后常規(guī)應(yīng)用縮宮素;術(shù)后米索前列醇組(B組,41例),在A組基礎(chǔ)上術(shù)后直腸放置米索前列醇;術(shù)中米索前列醇組(C組,43例),在A組基礎(chǔ)上術(shù)中含服米索前列醇。比較三組患者術(shù)中、術(shù)后0~2h、術(shù)后3~24h出血量,產(chǎn)后出血發(fā)生率及產(chǎn)后出血控制措施。結(jié)果:C組患者術(shù)中、術(shù)后0~2h、術(shù)后3~24h出血量分別為(240.12±64.09)ml、(51.26±15.04)ml、(61.26±19.80)ml,均比A組出血量少(P0.05);C組患者術(shù)中和術(shù)后0~2h出血量顯著低于B組(P0.05);B組患者術(shù)后0~2h、術(shù)后3~24h出血量顯著低于A組(P0.05)。C組患者產(chǎn)后出血發(fā)生率為4.65%,顯著低于A組(23.81%)(P0.05)。A組采取增加藥物止血措施的比例為19.05%,顯著高于C組(2.33%)(P0.05)。結(jié)論:術(shù)中含服米索前列醇聯(lián)合縮宮素防治二次剖宮產(chǎn)產(chǎn)后出血較術(shù)后給予米索前列醇和單純縮宮素給藥臨床效果好。
[Abstract]:Objective: to explore the clinical effect of misoprostol in the prevention and treatment of postpartum hemorrhage in patients with secondary cesarean section. Methods: 126 pregnant women undergoing secondary cesarean section were randomly divided into three groups: oxytocin group (group A, n = 42). Misoprostol was placed in rectum in group A (n = 41) and misoprostol in group C (n = 43). The amount of bleeding, the incidence of postpartum hemorrhage and the control measures of postpartum hemorrhage were compared between the three groups during operation, 0 hours after operation and 3 hours after operation. Results: the bleeding volume of group C was (240.12 鹵64.09) ml, (51.26 鹵15.04) ml, (61.26 鹵19.80) ml, which was less than that of group A (P 0.05). The amount of bleeding in group C was significantly lower than that in group B during and 2 hours after operation (P 0.05). The amount of bleeding in group B was significantly lower than that in group A at 0 h and 24 h after operation (P 0.05, the incidence of postpartum hemorrhage in). C group was 4.65%). It was significantly lower than that of group A (23.81%) (P 0.05). The proportion of increasing drug hemostatic measures in). Conclusion: the clinical effect of misoprostol combined with oxytocin in the prevention and treatment of postpartum hemorrhage after secondary cesarean section is better than that of misoprostol and oxytocin alone.
