【摘要】：目的了解健康體檢人群中女性宮頸非典型鱗狀細(xì)胞(ASCUS)的檢出情況并對(duì)其相關(guān)因素進(jìn)行分析。方法選擇2011年1月—2013年12月在貴陽(yáng)醫(yī)學(xué)院附屬醫(yī)院體檢中心接受體檢的健康婦女29 080例,均進(jìn)行常規(guī)婦科檢查、陰道分泌物檢查、宮頸脫落細(xì)胞檢查。分析不同年齡段婦女宮頸ASCUS檢出率,不同生殖道感染類(lèi)型(細(xì)菌性陰道病、滴蟲(chóng)性陰道炎、真菌性陰道炎、子宮頸糜爛樣改變、宮頸肥大并納氏囊腫)與宮頸ASCUS檢出率的關(guān)系及宮頸ASCUS的影響因素。結(jié)果宮頸ASCUS 359例,檢出率1.23%。不同年齡段婦女宮頸ASCUS檢出率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。20~29歲與50~59歲年齡段比較,差異有統(tǒng)計(jì)學(xué)意義(χ2=7.772,P=0.005);30~39歲與40~49歲年齡段比較,差異有統(tǒng)計(jì)學(xué)意義(χ2=8.596,P=0.003);30~39歲與50~59歲年齡段比較,差異有統(tǒng)計(jì)學(xué)意義(χ2=24.014,P0.001);40~49歲與50~59歲年齡段比較,差異有統(tǒng)計(jì)學(xué)意義(χ2=5.200,P=0.023);50~59歲與60~84歲年齡段比較,差異有統(tǒng)計(jì)學(xué)意義(χ2=4.053,P=0.044)。滴蟲(chóng)性陰道炎者宮頸ASCUS檢出率高于無(wú)滴蟲(chóng)性陰道炎者,子宮頸糜爛樣改變者宮頸ASCUS檢出率高于無(wú)子宮頸糜爛樣改變者,宮頸肥大并納氏囊腫者宮頸ASCUS檢出率高于無(wú)宮頸肥大并納氏囊腫者(P0.05)。細(xì)菌性陰道病、真菌性陰道炎者宮頸ASCUS檢出率與無(wú)細(xì)菌性陰道病、真菌性陰道炎者比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。Logistic回歸分析顯示,滴蟲(chóng)性陰道炎、子宮頸糜爛樣改變、宮頸肥大并納氏囊腫與宮頸ASCUS的發(fā)生有回歸關(guān)系(P0.05)。結(jié)論不同年齡段ASCUS檢出率不同,滴蟲(chóng)性陰道炎、宮頸糜爛樣改變、宮頸肥大并納氏囊腫是引起宮頸ASCUS的高危因素,因此定期宮頸細(xì)胞學(xué)檢查、及時(shí)治療滴蟲(chóng)性陰道炎及宮頸病變是預(yù)防宮頸癌的重要途徑。
[Abstract]:Objective to investigate the detection of atypical cervical squamous cell (ASCUS) in healthy people and analyze its related factors. Methods from January 2011 to December 2013, 29 080 healthy women underwent routine gynecological examination, vaginal secretion examination and cervical exfoliation cell examination in the physical examination center of Guiyang Medical College affiliated Hospital. The detection rate of cervical ASCUS in women of different ages and the changes of different types of reproductive tract infections (bacterial vaginitis, trichomonal vaginitis, fungal vaginitis, cervical chyme-like change) were analyzed. The relationship between cervical hypertrophy and the detection rate of cervical ASCUS and the influencing factors of cervical ASCUS. Results there were 359 cases of cervical ASCUS, the positive rate was 1.23%. There was a significant difference in the detection rate of cervical ASCUS among women of different ages (P0.05), and there was a significant difference between 20 and 59 years old (蠂 ~ 2, 7.772, P < 0.005), and there was a significant difference between 20 and 59 years old (蠂 ~ 2 = 7.772, P < 0.05). There was a significant difference between the age groups of 30 / 39 and 40 / 49 (蠂 ~ 2 = 8.596, P = 0.003), and between the age groups of 30 / 39 and 50 / 59 (蠂 ~ 2 / 24.014, P < 0.001), and there was a significant difference between the age groups of 30 / 39 and 50 / 59 (蠂 ~ 2 = 8.596, P < 0.001). There was a significant difference between the age groups of 40-49 years and 50-59 years (蠂 ~ 2-5.200, P < 0.023) and between 50-59-year-old and 60-84-year-old (蠂 ~ 2-4. 053, P < 0. 044). The detection rate of cervical ASCUS in patients with trichomonal vaginitis was higher than that in patients without trichomonal vaginitis, and the detection rate of cervical ASCUS in patients with cervical chymoidosis was higher than that in patients without cervical chymoidosis. The detection rate of cervical ASCUS was higher in patients with cervical hypertrophy complicated with Nessler's cyst than in those without cervical hypertrophy and Nessler's cyst (P0.05). There was no significant difference in the detection rate of cervical ASCUS between bacterial vaginosis, fungal vaginitis and non-bacterial vaginosis and fungal vaginitis (P0.05). Logistic regression analysis showed that trichomonas vaginitis and cervical chyme-like changes were found. There was a regression relationship between cervical hypertrophic cyst and cervical ASCUS (P0.05). Conclusion the detection rate of ASCUS in different age groups is different. Trichomonitis vaginitis, cervical erosion-like changes, cervical hypertrophy and Nessler's cyst are the high risk factors of cervical ASCUS. Timely treatment of trichomonal vaginitis and cervical lesions is an important way to prevent cervical cancer.
