【摘要】：目的分析高齡產(chǎn)婦剖宮產(chǎn)術(shù)后切口感染的危險因素,以期為臨床預(yù)防術(shù)后切口感染提供參考。方法回顧性分析2011年3月-2015年6月醫(yī)院接受剖宮產(chǎn)的1023例高齡產(chǎn)婦臨床資料,分析術(shù)后切口感染率及感染病原菌類型,采用Logistic回歸分析術(shù)后切口感染相關(guān)危險因素。結(jié)果 1023例高齡產(chǎn)婦術(shù)后切口感染118例,感染率為11.53%;118例感染患者送檢標(biāo)本分離病原菌114株,其中革蘭陰性菌86株占75.44%,革蘭陽性菌24株占21.05%,真菌4株占3.51%;單因素分析結(jié)果顯示:體質(zhì)量指數(shù)25kg/m2、術(shù)中出血量500ml、手術(shù)時間60min、侵入性操作4次,合并基礎(chǔ)疾病為術(shù)后切口感染的相關(guān)因素(P0.05);多因素Logistic回歸分析結(jié)果顯示:體質(zhì)量指數(shù)、術(shù)中出血量、基礎(chǔ)疾病、院內(nèi)侵入性操作次數(shù)等為高齡產(chǎn)婦術(shù)后切口感染的獨立危險因素。結(jié)論高齡產(chǎn)婦剖宮產(chǎn)術(shù)后切口感染的危險因素較多,醫(yī)護(hù)人員應(yīng)制定針對性的護(hù)理干預(yù)對策進(jìn)行預(yù)防,以期降低術(shù)后醫(yī)院切口感染率。
[Abstract]:Objective to analyze the risk factors of incision infection in elderly women after cesarean section. Methods the clinical data of 1023 elderly women undergoing cesarean section from March 2011 to June 2015 were analyzed retrospectively. The postoperative incision infection rate and pathogen types were analyzed. The risk factors of incision infection were analyzed by Logistic regression analysis. Results there were 118 cases of incision infection in 1023 elderly women, the infection rate was 11.53%. 114 strains of pathogens were isolated from 118 infected patients, of which 86 strains were Gram-negative bacteria (75.44%), 24 strains were Gram-positive bacteria (21.05%), and 4 strains were fungi (3.51%). The results of univariate analysis showed that the body mass index (BMI) was 25kg / m2, the intraoperative bleeding was 500ml, the operation time was 60 min, the invasive operation was 4 times, and the underlying diseases were the related factors of postoperative wound infection (P0.05). The results of multivariate Logistic regression analysis showed that the body mass index, intraoperative bleeding volume, basic diseases and the number of invasive operations in hospital were independent risk factors for postoperative incision infection in elderly women. Conclusion there are many risk factors of incision infection in elderly women after cesarean section. Medical staff should make appropriate nursing intervention measures to prevent incision infection in hospital after caesarean section operation in order to reduce the nosocomial incision infection rate.
