【摘要】：目的比較高強度聚焦超聲(HIFU)、甲氨蝶呤(MTX)聯(lián)合高強度聚焦超聲(MTX+HIFU)治療剖宮產(chǎn)瘢痕妊娠的臨床療效。方法將40例診斷明確的子宮下段瘢痕妊娠(CSP)患者分為兩組:HIFU組20例行HIFU治療,MTX+HIFU組20例行MTX 50mg,第1、3天連續(xù)肌肉注后再行HIFU治療,比較兩組患者治療后血β-HCG水平變化情況、清宮時陰道出血量,平均住院時間、月經(jīng)恢復正常時間、超聲提示術后子宮恢復情況,并進行隨訪。結果兩組患者療效比較,差異無統(tǒng)計學意義(P0.05),治療后血β-HCG恢復時間、陰道出血量、平均住院天數(shù)、月經(jīng)恢復正常時間方面比較差異有統(tǒng)計學意義(P0.05),其中,HIFU組血β-HCG恢復時間長、平均住院時間短、陰道出血量較多,MTX+HIFU組血β-HCG恢復時間短、平均住院時間稍長、陰道出血量少。兩組患者均無嚴重不良反應發(fā)生。結論 HIFU聯(lián)合MTX治療CSP的療效優(yōu)于HIFU治療。
[Abstract]:Objective to compare the clinical effect of high intensity focused ultrasound (HIFU) (HIFU), methotrexate (MTX) combined with high intensity focused ultrasound (MTX HIFU) in the treatment of cesarean scar pregnancy. Methods 40 patients with (CSP) of lower uterine scar pregnancy were divided into two groups: HIFU group (20 cases) were treated with HIFU and, MTX HIFU group (20 cases) were treated with MTX 50 mg, and then treated with HIFU after 3 days of continuous intramuscular injection. The changes of serum 尾-HCG, vaginal bleeding, average hospitalization time, menstrual recovery time and uterine recovery after operation were compared between the two groups. Results there was no significant difference in the curative effect between the two groups (P0.05). There were significant differences in the recovery time of serum 尾-HCG, vaginal bleeding volume, average hospitalization days, menstrual recovery time after treatment (P0.05). Among them, the recovery time of blood 尾-HCG in HIFU group was long, the average hospitalization time was short, the recovery time of blood 尾-HCG in HIFU group was shorter than that in, MTX HIFU group, the average hospitalization time was a little longer, and the amount of vaginal bleeding was less. No serious adverse reactions occurred in both groups. Conclusion HIFU combined with MTX is superior to HIFU in the treatment of CSP.
