【摘要】：目的:探討高齡孕婦對(duì)2種不同唐氏產(chǎn)前診斷方案的需求,并對(duì)這2種方案進(jìn)行經(jīng)濟(jì)學(xué)評(píng)價(jià)。方法:選擇2015年3月1日至2016年3月31日孕20+6周前在深圳市婦幼保健院產(chǎn)科就診的單胎高齡孕婦4104例,由孕婦自主選擇先行無(wú)創(chuàng)胎兒DNA檢測(cè)(NIPT),NIPT高風(fēng)險(xiǎn)再行羊膜腔穿刺(方案1)或直接羊膜腔穿刺(方案2)。比較孕婦對(duì)2種方案的需求及其影響因素,并對(duì)2種方案進(jìn)行經(jīng)濟(jì)學(xué)評(píng)價(jià)。結(jié)果:選擇方案1的比率明顯高于方案2[51.34%(2107/4104)vs 7.65%(314/4104),χ~2=1883.431,P0.001]。Logistic回歸分析顯示,孕婦年齡、傳統(tǒng)唐氏篩查結(jié)果和胎兒頸部透明層厚度(NT)是影響孕婦選擇的因素。與方案2相比,方案1可以節(jié)省67.21%的總費(fèi)用。結(jié)論:高齡孕婦對(duì)NIPT有巨大的需求,且高齡孕婦采用NIPT篩查陽(yáng)性后再行羊膜腔穿刺術(shù)可節(jié)省醫(yī)療費(fèi)用,建議在充分知情同意的基礎(chǔ)上可將NIPT用于高齡孕婦的一線篩查,尤其是適用于年齡較輕、傳統(tǒng)唐氏篩查非高風(fēng)險(xiǎn)、NT正常的高齡孕婦。
[Abstract]:Objective: to investigate the need of two different Down's prenatal diagnosis schemes for elderly pregnant women and to evaluate them economically. Methods: from March 1, 2015 to March 31, 2016, 4104 elderly single born pregnant women who had been admitted to the obstetrics department of Shenzhen Maternal and Child Health Care Hospital were selected for the detection of (NIPT), by non-invasive fetal DNA. NIPT underwent high risk amniocentesis (option 1) or direct amniocentesis (option 2). The demands of two schemes and their influencing factors were compared, and the economic evaluation of the two schemes was carried out. Results: the ratio of option 1 was significantly higher than that of option 2 [51.34% (2107 / 4104) vs 7.65% (314 / 4104), 蠂 ~ (2 +) 1883.431 / P0.001]. Logistic regression analysis showed that the age of pregnant women was significantly higher than that of option 2 [51.34% (2107 / 4104) vs 7.65% (314 / 4104)]. The results of traditional Down screening and the thickness of transparent layer of fetal neck (NT) are the factors influencing the selection of pregnant women. Compared with scenario 2, scheme 1 can save 67.21% of total cost. Conclusion: there is a great demand for NIPT in elderly pregnant women, and amniocentesis after NIPT positive screening can save medical cost. It is suggested that NIPT can be used for first-line screening of elderly pregnant women on the basis of fully informed consent. Especially suitable for younger, traditional Down screening non-high risk, NT normal elderly pregnant women.
