【摘要】：目的探討婦科手術(shù)后腸梗阻發(fā)生的危險因素。方法回顧分析35例北京大學(xué)人民醫(yī)院診治的術(shù)后腸梗阻的婦科住院患者的臨床資料,并與具有相同年齡同期手術(shù)但術(shù)后未發(fā)生腸梗阻的140例婦科患者進(jìn)行對比分析(按1∶4比例)。結(jié)果術(shù)后腸梗阻組與非腸梗阻組相關(guān)臨床資料進(jìn)行單因素分析顯示:婦科惡性腫瘤、開腹手術(shù)、切除盆腔淋巴結(jié)、合并盆腔粘連、手術(shù)操作時間長、術(shù)后使用鎮(zhèn)痛泵、術(shù)后合并低鉀血癥與婦科手術(shù)后腸梗阻相關(guān)。多因素分析顯示:手術(shù)時間是婦科手術(shù)后腸梗阻的獨(dú)立危險因素。結(jié)論婦科惡性腫瘤、開腹手術(shù)、切除盆腔淋巴結(jié)、合并盆腔粘連、手術(shù)操作時間長、術(shù)后使用鎮(zhèn)痛泵、術(shù)后合并低鉀血癥的婦科手術(shù)患者術(shù)后發(fā)生腸梗阻的風(fēng)險增加,應(yīng)提高警惕和加強(qiáng)術(shù)后管理,其中手術(shù)操作時間與術(shù)后腸梗阻的發(fā)生最為密切,縮短手術(shù)時間是減少術(shù)后腸梗阻發(fā)生的最為有效的措施。
[Abstract]:Objective to investigate the risk factors of intestinal obstruction after gynecological surgery. Methods the clinical data of 35 cases of gynecological patients with intestinal obstruction diagnosed and treated in Peking University people's Hospital were retrospectively analyzed. A comparative analysis was made between 140 gynecological patients with the same age but no postoperative intestinal obstruction (at 1:4). Results the clinical data of postoperative intestinal obstruction group and non-intestinal obstruction group were analyzed by single factor analysis. The results showed that gynecologic malignant tumor, open operation, pelvic lymph node resection, pelvic adhesion, long operation time and postoperative analgesia pump were used. Postoperative hypokalemia was associated with intestinal obstruction after gynecological surgery. Multivariate analysis showed that operative time was an independent risk factor for intestinal obstruction after gynecological surgery. Conclusion the risk of intestinal obstruction in gynecologic malignant tumor patients with postoperative hypokalemia is increased by open surgery, pelvic lymph node resection, pelvic adhesion, long operation time, postoperative analgesia pump and postoperative hypokalemia. The operative time is the most closely related to the occurrence of postoperative intestinal obstruction, and shortening the operative time is the most effective measure to reduce the incidence of postoperative intestinal obstruction.
