【摘要】：目的:探究地塞米松治療重度妊娠高血壓合并血小板減少綜合征的臨床治療效果。方法:選擇120例重度妊娠高血壓合并血小板減少綜合征孕婦,分為對(duì)照組和觀察組各60例,觀察組根據(jù)孕婦的血小板計(jì)數(shù)情況給予分組,其中輕度組18例,中度組20例,重度組22例。對(duì)所有孕婦采用解痙攣以及鎮(zhèn)靜、降壓、擴(kuò)容、利尿等治療措施,對(duì)照組采用硫酸鎂治療,觀察組給予地塞米松治療,最后對(duì)所有孕婦的臨床表現(xiàn)以及預(yù)后情況進(jìn)行比較分析。結(jié)果:觀察組的并發(fā)癥以及圍產(chǎn)兒死亡情況與對(duì)照組相比差異有統(tǒng)計(jì)學(xué)意義(P0.05),且重度組孕婦的妊娠并發(fā)癥以及圍產(chǎn)兒死亡率高于中度組與輕度組(P0.05);在產(chǎn)婦預(yù)后中,輕度組的ALT、肝功能以及血小板恢復(fù)時(shí)間優(yōu)于中度組與重度組(P0.05)。結(jié)論:采用地塞米松對(duì)重度妊娠高血壓合并血小板減少綜合征孕婦進(jìn)行治療,適時(shí)終止妊娠,能夠有效降低并發(fā)癥與圍產(chǎn)兒死亡率,改善母嬰預(yù)后,可以在臨床醫(yī)學(xué)中得以廣泛地推廣應(yīng)用。
[Abstract]:Objective: to investigate the clinical effect of dexamethasone in the treatment of severe pregnancy-induced hypertension complicated with thrombocytopenia syndrome. Methods: 120 pregnant women with severe pregnancy-induced hypertension complicated with thrombocytopenia syndrome were divided into two groups: control group (n = 60) and observation group (n = 60). Severe group 22 cases. All pregnant women were treated with antispasticity, sedation, hypotension, dilatation, diuretic therapy, the control group was treated with magnesium sulfate, the observation group was treated with dexamethasone, and the clinical manifestations and prognosis of all pregnant women were compared and analyzed. Results: the complications and perinatal mortality in the observation group were significantly higher than those in the control group (P0.05), and the pregnancy complications and perinatal mortality in the severe group were higher than those in the moderate group and the mild group (P0.05). In maternal prognosis, ALT, liver function and platelet recovery time in mild group were better than those in moderate group and severe group (P0.05). Conclusion: using dexamethasone to treat pregnant women with severe pregnancy-induced hypertension and thrombocytopenia syndrome and terminating pregnancy timely can effectively reduce complications and perinatal mortality and improve the prognosis of mother and infant. It can be widely used in clinical medicine.
