【摘要】：目的探討左炔諾孕酮宮內(nèi)緩釋系統(tǒng)與小劑量米非司酮在子宮腺肌病治療中的治療效果。方法選擇2015年1月—2016年12月我院婦產(chǎn)科接受治療的100例子宮腺肌病患者,隨機(jī)分為左炔諾孕酮宮內(nèi)緩釋系統(tǒng)組和米非司酮組,每組50例,分別接受子宮放置左炔諾孕酮宮內(nèi)緩釋系統(tǒng)與口服小劑量米非司酮進(jìn)行治療。檢查兩組患者治療前、后子宮體積、月經(jīng)情況及骨密度,采用視覺(jué)模擬評(píng)分法(VAS)評(píng)估治療前、后痛經(jīng)疼痛情況,并觀察兩組患者臨床療效。結(jié)果治療前和治療后,兩組月經(jīng)量、月經(jīng)周期、經(jīng)期、VAS得分、骨密度及子宮體積分別比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。兩組治療后與治療前比較,月經(jīng)量減少,月經(jīng)周期延長(zhǎng),經(jīng)期縮短,VAS得分降低,子宮體積減小,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者總有效率及不良反應(yīng)發(fā)生率比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論左炔諾孕酮宮內(nèi)緩釋系統(tǒng)與小劑量米非司酮在子宮腺肌病治療中均具有較為理想的治療效果,可有效改善患者月經(jīng)情況及痛經(jīng)癥狀,子宮體積減小,療效確切,安全性高。
[Abstract]:Objective to evaluate the efficacy of levonorgestrel in the treatment of adenomyosis. Methods from January 2015 to December 2016, 100 patients with adenomyosis were randomly divided into levonorgestrel intrauterine release system group and mifepristone group with 50 cases in each group. Intrauterine delivery of levonorgestrel and oral low-dose mifepristone were performed respectively. The volume of uterus, menstruation and bone mineral density were examined before and after treatment in both groups. Visual analogue score (VAS) was used to evaluate the dysmenorrhea pain before and after treatment, and to observe the clinical efficacy of the two groups. Results before and after treatment, there was no significant difference in menstrual volume, menstrual cycle, menstrual period, VAS score, bone mineral density and uterine volume between the two groups (P0.05). After treatment, compared with before treatment, menstrual volume decreased, menstrual cycle prolonged, menstrual period shortened, VAS score decreased, uterine volume decreased, the difference was statistically significant (P0.05). There was no significant difference in the total effective rate and the incidence of adverse reactions between the two groups (P0.05). Conclusion levonorgestrel intrauterine slow-release system and low-dose mifepristone are effective in the treatment of adenomyosis, which can effectively improve menstruation and dysmenorrhea symptoms, reduce uterine volume, and have high efficacy and safety.
