【摘要】：目的探討經(jīng)后路子宮切除與傳統(tǒng)子宮切除術(shù)治療穿透性胎盤植入的手術(shù)并發(fā)癥和術(shù)后情況。方法回顧性分析2010年12月至2014年5月在廣州醫(yī)科大學(xué)第三附屬醫(yī)院診斷為穿透性胎盤植入并行圍產(chǎn)期經(jīng)后路子宮切除術(shù)患者18例,以及傳統(tǒng)子宮切除術(shù)治療穿透性胎盤植入患者30例,比較兩組孕產(chǎn)婦的一般情況、手術(shù)并發(fā)癥及術(shù)后情況。結(jié)果同期確診胎盤植入420例,其中48例因發(fā)生危及生命的產(chǎn)后出血,需要行圍產(chǎn)期子宮切除術(shù)(11.4%)。兩組均無一例產(chǎn)婦死亡。經(jīng)后路子宮切除術(shù)組術(shù)中無一例發(fā)生膀胱損傷;而傳統(tǒng)子宮切除術(shù)組發(fā)生了2例術(shù)中膀胱損傷(6.7%);經(jīng)子宮后路切除術(shù)組手術(shù)后輸血量及ICU入住天數(shù)與傳統(tǒng)子宮切除術(shù)組相比差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論相對(duì)于傳統(tǒng)的圍產(chǎn)期子宮切除術(shù),經(jīng)后路子宮切除術(shù)在治療胎盤植入,尤其是浸潤(rùn)至膀胱的穿透性胎盤植入,可能是更好的手術(shù)選擇。
[Abstract]:Objective to investigate the postoperative complications of transposterior hysterectomy and traditional hysterectomy in the treatment of penetrating placenta accreta. Methods from December 2010 to May 2014, 18 patients diagnosed as penetrating placenta accreta and perinatal hysterectomy in the third affiliated Hospital of Guangzhou Medical University were analyzed retrospectively. Thirty cases of penetrating placenta accreta were treated with traditional hysterectomy. Results 420 cases of placenta accreta were diagnosed in the same period, of which 48 cases needed perinatal hysterectomy due to life-threatening postpartum hemorrhage (11.4%). There was no maternal death in both groups. No bladder injury occurred in the posterior hysterectomy group, while in the traditional hysterectomy group, bladder injury occurred in 2 cases (6.7%). The amount of blood transfusion and the days of ICU stay in the posterior hysterectomy group were significantly different from those in the traditional hysterectomy group (P0.05). Conclusion compared with the traditional perinatal hysterectomy, transposterior hysterectomy may be a better choice in the treatment of placenta accreta, especially penetrating placenta accreta which infiltrates the bladder.
