【摘要】：目的探討超聲引導(dǎo)下甲氨蝶呤(methotrexate,MTX)局部注射聯(lián)合米非司酮治療未破裂型輸卵管妊娠的療效。方法 2013年1月~2014年2月對120例未破裂型輸卵管妊娠,在B超引導(dǎo)下在未破裂型輸卵管妊娠囊內(nèi)注射MTX1 mg/kg,同時(shí)口服米非司酮75 mg,2次/d,共4 d。結(jié)果超聲引導(dǎo)下112例1次穿刺成功,8例2次穿刺成功。80例(66.7%)治療后第4天β-h CG下降15%,10例(8.3%)無明顯變化,30例(25.0%)上升,但15%。105例治療后第7天β-h CG下降15%,治療后第14天進(jìn)一步下降,1個(gè)月后降為陰性,腹痛明顯緩解至消失,陰道流血停止,包塊縮小或消失,治療成功率87.5%(105/120)。住院費(fèi)用1208.5~2722.6元,平均1859.4元。余15例腹腔鏡手術(shù)治療。結(jié)論超聲引導(dǎo)下MTX局部注射聯(lián)合米非司酮治療未破裂型輸卵管妊娠簡便、安全。
[Abstract]:Objective to investigate the effect of local injection of methotrexate (methotrexate,MTX) combined with mifepristone on unruptured tubal pregnancy. Methods from January 2013 to February 2014, 120 cases of unruptured tubal pregnancy were treated with intrathecal injection of MTX1 mg/kg, and oral mifepristone 75 times per day for 4 days under the guidance of B-ultrasound. Results under the guidance of ultrasound, 112 cases were successfully punctured once, 8 cases 2 times successfully, 80 cases (66.7%) were successfully punctured, 10 cases (8.3%) of 尾 -h CG decreased in 15 cases (8.3%) on the 4th day after treatment, 30 cases (25.0%) increased. However, 尾 -h CG decreased by 15% on the 7th day after treatment, further decreased on the 14th day after treatment, decreased to negative one month later, abdominal pain obviously relieved to disappear, vaginal bleeding stopped, and mass decreased or disappeared. The success rate of treatment was 87.5% (105 / 120). The hospitalization expenses were 1208.5 yuan, 2722.6 yuan, with an average of 1859.4 yuan. The remaining 15 cases were treated by laparoscopic surgery. Conclusion Ultrasound-guided local injection of MTX combined with mifepristone in the treatment of unruptured tubal pregnancy is simple and safe.
