【摘要】：目的：報道腹膜代陰道成形術的改良術式并分析其優(yōu)勢；通過回顧性分析評價該項改良術式的效果,得出術后放置陰道模具和規(guī)律性生活的最佳時間,從而為MRKH患者的手術治療提供可靠、安全的方法。材料和方法：調查2006-2013年間22例在我院接受腹膜代陰道成形術患者的手術時間、出血量、術中陰道長度、術后平均住院日以及FSFI量表得分情況對該項術式進行評價。經電話回訪,16位患者完成了FSFI量表。對照組為32名根據(jù)年齡按照例數(shù)2：1匹配的已婚健康女性。采用Microsoft Excel輸入數(shù)據(jù),所有資料采用SPSS 19.0進行處理。統(tǒng)計方法采用描述性統(tǒng)計分析、配對樣本t檢驗及回歸分析,計算決定系數(shù)R2,所有均數(shù)均表達為平均數(shù)±標準差。結果：(1)患者接受手術的平均年齡為23(17-31)歲,手術時間為(93±25.1)min,術中出血量(81.3±44.5)ml,術中陰道長度(10.4±1.4)cm,術后平均住院日為(8.6±1.9)d。(2)22例手術均順利完成,術后隨訪16例,回訪率為72.8%。FSFI量表的平均得分為25.28±2.61,對照組為25.61±2.79,二者無統(tǒng)計學差異。(3)患者術后放置陰道模具的時間為(12.5±5.6)月,術后開始性生活的時間為(8.0±3.5)月,通過比較決定系數(shù)及回歸方程分析,得出術后放置陰道模具的最佳時間為6.9月,開始規(guī)律性生活的最佳時間為5.6月。結論：改良式腹腔鏡下腹膜代陰道成形術是對MRKH患者進行的一種安全、方便的手術方式,患者術后可獲得可靠的性生活滿意度。
[Abstract]:Objective: to report the modified procedure of vaginoplasty of peritoneum and analyze its advantages. Through retrospective analysis and evaluation of the effect of the modified operation, the best time for placement of vaginal mould and regular life after operation was obtained, thus providing a reliable and safe method for the surgical treatment of patients with MRKH. Materials and methods: the operative time, bleeding volume, length of vagina during operation, average hospitalization days after operation and the score of FSFI scale were investigated in 22 patients undergoing vaginoplasty in our hospital from 2006 to 2013. By telephone interview, 16 patients completed the FSFI scale. The control group consisted of 32 healthy married women who matched at 2:1 according to their age. Microsoft Excel was used to input data and all data were processed by SPSS 19.0. Descriptive statistical analysis, paired sample t-test and regression analysis were used to calculate the determination coefficient R2. All mean values were expressed as mean 鹵standard deviation. Results: (1) the mean age of patients undergoing surgery was 23 (17-31) years, and the operative time was (93 鹵25.1) min,. The amount of bleeding during operation was (81.3 鹵44.5) ml,. The length of vagina was (10.4 鹵1.4) cm,. The average postoperative hospitalization days were (8.6 鹵1.9) d. (2), 22 cases were successfully completed, 16 cases were followed up, the average score of 72.8%.FSFI scale was 25.28 鹵2.61, and that of the control group was 25.61 鹵2.79, while that of the control group was 25.61 鹵2.79. There was no significant difference between the two groups. (3) the time of placement of vaginal mould was (12.5 鹵5.6) months, and the time of starting sexual life was (8.0 鹵3.5) months. The optimal time for placement of vaginal mould was 6.9 months and the best time for regular life was 5.6 months. Conclusion: modified laparoscopic retroperitoneal vaginoplasty is a safe and convenient method for MRKH patients.
