【摘要】：目的:探討綜合消腫療法(Complex Decongestion Therapy,CDT)治療婦科惡性腫瘤術(shù)后單側(cè)下肢淋巴水腫的療效以及其對(duì)生活質(zhì)量的影響。材料與方法:自2016年1月起,共收治婦科惡性腫瘤術(shù)后引起的單側(cè)下肢淋巴水腫患者42例,其中包括子宮內(nèi)膜癌24例,子宮頸癌10例和卵巢癌8例;按照健患側(cè)下肢周徑的差異,將患者分為3組:輕度組18例,中度組14例,重度組10例。所有患者均被給予CDT治療,每天一次,20天為一個(gè)療程,共治療2個(gè)療程,并囑咐同一醫(yī)護(hù)人員用同一規(guī)格的卷尺測(cè)量CDT治療前后的患肢周徑來觀察療效。結(jié)果:1.經(jīng)過2周期的CDT治療后,所有患者下肢水腫的周徑值均低于治療前(P0.05)。2.隨訪1年時(shí)間內(nèi)觀察患者治療情況,均未見明顯復(fù)發(fā)及加重。在3組中,中、重度水腫組均較輕度水腫組下降明顯,且重度水腫組最為明顯。3.所有患者治療后各項(xiàng)生活質(zhì)量指標(biāo)評(píng)分均較治療前提高(P0.05)。結(jié)論:1.CDT是治療婦科惡性腫瘤術(shù)后下肢淋巴水腫安全有效的治療方法,且能夠長期維持治療效果。2.CDT適用于手術(shù)后各期下肢繼發(fā)性淋巴水腫,包括重度淋巴水腫。3.CDT能夠提高下肢淋巴水腫患者的生活質(zhì)量。
[Abstract]:Objective: to investigate the effect of comprehensive detumescence therapy (Complex Decongestion Therapy,CDT) on unilateral lymphedema of lower extremity after operation of gynecological malignant tumor and its effect on quality of life. Materials and methods: from January 2016, 42 patients with unilateral lower limb lymphedema caused by gynecological malignant tumors were treated, including 24 cases of endometrial carcinoma, 10 cases of cervical cancer and 8 cases of ovarian cancer. The patients were divided into 3 groups according to the difference of the circumference of the healthy side of lower extremities: light group (18 cases), moderate group (14 cases) and severe group (10 cases). All patients were treated with CDT, once a day, 20 days as a course of treatment, a total of 2 courses of treatment, and told the same medical staff to measure the circumference of the affected limb with the same specification tape measure before and after CDT treatment to observe the curative effect. The result is 1: 1. After 2 cycles of CDT treatment, the circumference of edema in all patients was lower than that before treatment (P0.05). No recurrence or aggravation was found during the 1-year follow-up. In the three groups, severe edema group was significantly lower than that of mild edema group, and severe edema group was the most obvious. All patients after treatment of quality of life scores were higher than before treatment (P0.05). Conclusion: 1.CDT is a safe and effective method for the treatment of lower extremity lymphedema after operation for gynecological malignant tumors, and can maintain the therapeutic effect for a long time. 2.CDT is suitable for the treatment of secondary lymphedema of lower extremity after operation. Including severe lymphedema. 3.CDT can improve the quality of life in patients with lower extremity lymphedema.
