【摘要】：目的分析子宮腺肌病惡變的臨床及病理學(xué)特點(diǎn)。方法回顧性分析北京大學(xué)人民醫(yī)院9例子宮腺肌病惡變患者的臨床特點(diǎn)、病理學(xué)特征及診治預(yù)后情況。結(jié)果患者平均年齡為(48.7±5.1)歲,7例未絕經(jīng)。主要臨床表現(xiàn)為陰道不規(guī)則流血。術(shù)前都沒(méi)有獲得診斷。術(shù)中冰凍病理發(fā)現(xiàn)3例腺肌癥惡變。惡變類(lèi)型是子宮內(nèi)膜樣腺癌8例,透明細(xì)胞癌1例。免疫組化7例ER(+),6例PR(+),5例P53(+),4例CD10(+)。FIGO分期8例為ⅠA期,1例為ⅠB期。8例行全面分期手術(shù),1例行全子宮+雙側(cè)附件切除術(shù)。4例采用鉑類(lèi)為基礎(chǔ)的聯(lián)合化療,1例口服孕激素。8例患者無(wú)瘤生存期(60.9±19.8)個(gè)月。1例失訪。結(jié)論子宮腺肌病惡變患者發(fā)病較年輕,Ⅰ期患者多。術(shù)前診斷困難,術(shù)中冰凍病理有一定價(jià)值,需要術(shù)后病理確診。治療參照子宮內(nèi)膜腺癌和卵巢上皮癌,預(yù)后比較好。
[Abstract]:Objective to analyze the clinical and pathological features of malignant changes of adenomyosis. Methods the clinical features, pathological features, diagnosis and treatment and prognosis of 9 patients with adenomyosis and malignancy in the people's Hospital of Peking University were retrospectively analyzed. Results the average age of the patients was (48.7 鹵5.1) years. The main clinical manifestations were irregular vaginal bleeding. No diagnosis was made before the operation. Intraoperative frozen pathology found 3 cases of adenomyosis malignant change. The malignant type was endometrial adenocarcinoma in 8 cases and clear cell carcinoma in 1 case.
