激活的蛋白激酶C受體1 RACK1在宮頸上皮內(nèi)瘤變及宮頸癌中的表達(dá)及意義

【摘要】:目的:1.檢測(cè)激活的蛋白激酶C受體1(receptor for activated C kinase 1,RACK1)在正常宮頸、宮頸上皮內(nèi)瘤變及宮頸鱗癌組織中的表達(dá)。2.結(jié)合高危型人乳頭瘤病毒(high risk human papillomavirus,HR-HPV)檢測(cè)結(jié)果,分析RACK1與HR-HPV感染的相關(guān)性。方法:隨機(jī)選取于2015年10月-2016年10月在山西醫(yī)科大學(xué)附屬第二醫(yī)院婦產(chǎn)科進(jìn)行診治的患者,其中行陰道鏡活檢、宮頸錐切術(shù)、子宮全切術(shù)、或?qū)m頸癌手術(shù)后經(jīng)病理證實(shí)為正常宮頸組織(正常對(duì)照)15例,早期宮頸鱗癌(cervical squamous cell carcinoma,CSCC)的標(biāo)本15例,宮頸上皮內(nèi)瘤變I級(jí)(Cervical intraepithe lia neoplasisa grade I,CINI)、宮頸上皮內(nèi)瘤變II級(jí)(Cervical intraepithel ia neoplasisa grade II,CINII)、宮頸上皮內(nèi)瘤變III級(jí)(Cervical intraepit helia neoplasisa grade III,CINIII)標(biāo)本各30例。使用免疫組織化學(xué)SP法檢測(cè)上述所有不同種類石蠟切片標(biāo)本中RACK1表達(dá)情況,同時(shí)使用免疫印記western blot法分別檢測(cè)RACK1在15例早期宮頸鱗癌及15例正常宮頸組織新鮮組織標(biāo)本中的表達(dá)情況。同時(shí)取上述病人的宮頸分泌物標(biāo)本采用HPV基因分型測(cè)定檢測(cè)上述組織中HR-HPV感染情況,并對(duì)HR-HPV與RACK1的結(jié)果進(jìn)行對(duì)照分析。從而探討RAC K1蛋白在正常宮頸、宮頸上皮內(nèi)瘤變及宮頸鱗癌組織中的表達(dá)與HR-HPV感染的相關(guān)性。結(jié)果:1.免疫組化法檢測(cè)結(jié)果顯示:RACK1蛋白在正常宮頸組織、CINI級(jí)組織,CINII級(jí)組織,CINIIII級(jí)組織和CSCC組織中的陽(yáng)性表達(dá)率逐漸增高,依次為13.33%(2/15),30%(9/30),53.33%(16/30),80%(24/30)和100%(15/15),可見(jiàn)每組之間呈現(xiàn)逐漸升高的陽(yáng)性表達(dá)率,五組間有明顯的統(tǒng)計(jì)學(xué)差異(P0.05)。免疫印跡法檢測(cè)結(jié)果顯示:RACK1蛋白在宮頸鱗癌組和正常宮頸組中的相對(duì)表達(dá)水平分別為:0.89±0.07,0.29±0.01。宮頸鱗癌組與正常宮頸組陰性對(duì)照相比,RACK1的相對(duì)表達(dá)量增加了3.07倍,差異有統(tǒng)計(jì)學(xué)意義(P0.05)2.HR-HPV在正常宮頸組織、CINI級(jí)組織,CINII級(jí)組織,CINIIII級(jí)組織和CSCC組織中的陽(yáng)性表達(dá)率分別為13.33%(2/15)、60%(18/30)、76.67%(23/30),90%(27/30)以及100%(15/15),四組間感染率的差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。且經(jīng)過(guò)Spearman相關(guān)性分析后顯示,HP-HPV的感染同RACK1的陽(yáng)性表達(dá)呈正相關(guān)性,相關(guān)系數(shù)r=0.266。結(jié)論:1.RACK1的陽(yáng)性表達(dá)率均隨著宮頸病變程度加重逐漸升高。且HR-HPV的陽(yáng)性表達(dá)率均隨著宮頸病變程度加重逐漸升高。2.RACK1的表達(dá)與HPV的感染呈正相關(guān),表明RACK1蛋白可能在HPV致宮頸癌過(guò)程中起重要的作用。
[Abstract]:Purpose 1. The expression of activated protein kinase C receptor 1 (receptor for activated C kinase 1 rack1 was detected in normal cervix, cervical intraepithelial neoplasia and cervical squamous cell carcinoma. Combined with high risk human papillomavirus (high risk human papillomavirus,HR-HPV) detection results, the relationship between RACK1 and HR-HPV infection was analyzed. Methods: randomly selected patients from October 2015 to October 2016 in the second affiliated Hospital of Shanxi Medical University for gynecology and obstetrics, including colposcopy biopsy, cervical conization, and total hysterectomy. Or 15 cases of normal cervical tissue (normal control) and 15 cases of early cervical squamous cell carcinoma (cervical squamous cell carcinoma,CSCC) confirmed by pathology after operation of cervical carcinoma. There were 30 cases of cervical intraepithelial neoplasia (Cervical intraepithe lia neoplasisa grade I), 30 cases of cervical intraepithelial neoplasia (II grade (Cervical intraepithel ia neoplasisa grade II,CINII) and 30 cases of cervical intraepithelial neoplasia (III grade (Cervical intraepit helia neoplasisa grade III,CINIII). Immunohistochemical SP method was used to detect the expression of RACK1 in all kinds of paraffin sections. The expression of RACK1 in 15 cases of early squamous cell carcinoma of cervix and 15 cases of fresh tissue of normal cervix were detected by western blot. HPV genotyping was used to detect the infection of HR-HPV in the above tissues, and the results of HR-HPV and RACK1 were compared and analyzed. To investigate the expression of RAC K1 protein in normal cervix, cervical intraepithelial neoplasia and cervical squamous cell carcinoma (SCC) and its correlation with HR-HPV infection. The result is 1: 1. The results of immunohistochemical staining showed that the positive expression rate of 1% RACK1 protein in CINI grade III and CSCC tissues of normal cervix was increased gradually, which was 13.33% (2 / 15) 30% (9 / 30) and 80% (24 / 30) and 100% (15 / 15) respectively. There was significant statistical difference among the five groups (P0.05). The results of Western blotting showed that the relative expression level of 1% RACK1 protein in cervical squamous cell carcinoma group and normal cervical cancer group was 0.89 鹵0.07 and 0.29 鹵0.01, respectively. The relative expression of rack1 in cervical squamous cell carcinoma group was 3.07 times higher than that in normal cervical cancer group. The positive expression rates of 2.HR-HPV were 13.33% (2 / 15) and 60% (18 / 30) respectively in normal cervical tissue of CINI grade III and CSCC, respectively. The positive rates of 2.HR-HPV were 76.67% (23 / 30), 90% (27 / 30) and 100% (15 / 15), respectively. There was significant difference in the infection rate among the four groups (P0.05). Spearman correlation analysis showed that the infection of HP-HPV was positively correlated with the positive expression of RACK1, and the correlation coefficient was 0.266. Conclusion 1. The positive expression rate of RACK1 increased with the severity of cervical lesions. The positive expression rate of HR-HPV increased with the severity of cervical lesions. 2. The expression of RACK1 was positively correlated with the infection of HPV, indicating that RACK1 protein may play an important role in the process of cervical cancer induced by HPV.
