【摘要】：目的:探討改良盆底重建術(shù)治療盆腔器官脫垂(POP)的臨床實(shí)用性。方法:選取59例POP患者,其中19例采用改良盆底重建術(shù)(A組),20例采用陰式子宮全切除術(shù)+陰道前壁"斜拉橋"式修補(bǔ)術(shù)(B組);20例采用傳統(tǒng)陰式子宮全切除術(shù)+陰道前壁修補(bǔ)術(shù)(C組)。比較3組患者的圍手術(shù)期各項(xiàng)指標(biāo)情況及術(shù)后復(fù)發(fā)、陰道深度、性生活保持情況及疼痛不適等并發(fā)癥情況。結(jié)果:(1)3組患者的手術(shù)時(shí)間、術(shù)中出血量、術(shù)后病率、術(shù)后性生活保持率及術(shù)后隨訪主觀復(fù)發(fā)率比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05);3組的術(shù)后留置尿管時(shí)間及客觀復(fù)發(fā)率兩兩比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05),A組的留置尿管時(shí)間最短,C組的客觀復(fù)發(fā)率較高。C組術(shù)后的平均陰道深度淺于A、B組,差異有統(tǒng)計(jì)學(xué)意義(P=0.000);但后兩組比較差異無統(tǒng)計(jì)學(xué)意義。3組患者均無嚴(yán)重并發(fā)癥發(fā)生。結(jié)論:3種術(shù)式均能短期內(nèi)解決POP問題,改良盆底重建術(shù)術(shù)式簡(jiǎn)單易操作,可保持盆底結(jié)構(gòu)及功能的完整性,短期內(nèi)具有較好的療效,但遠(yuǎn)期療效尚待循證。
[Abstract]:Objective: to investigate the clinical practicability of modified pelvic floor reconstruction in the treatment of pelvic organ prolapse (POP). Methods: 59 patients with POP were selected. Among them, 19 cases were treated with modified pelvic floor reconstruction (group A) and 20 cases were treated with vaginal anterior wall repair (group B), 20 cases with traditional vaginal hysterectomy (group C). The perioperative indexes, postoperative recurrence, vaginal depth, sex life, pain and discomfort were compared among the three groups. Results: (1) the time of operation, the amount of blood lost during operation, the rate of disease after operation, There was no significant difference between the retention rate of postoperative sexual life and the subjective recurrence rate of postoperative follow-up (P0.05) the time of indwelling urethral catheter and the objective recurrence rate of three groups were significantly different (P0.05) the time of indwelling urethral catheter in group A was the shortest. The objective recurrence rate in group C was higher than that in group A. the average depth of vagina in group C was lighter than that in group A (P < 0.05). The difference was statistically significant (P < 0. 000), but there was no significant difference between the latter two groups. Conclusion all three methods can solve the POP problem in a short time. The modified pelvic floor reconstruction is simple and easy to operate and can maintain the integrity of the structure and function of the pelvic floor. It has a good effect in the short term, but the long-term effect remains to be documented.
