【摘要】：目的:探討既往自然流產(chǎn)次數(shù)與患者第3日胚胎質(zhì)量的關(guān)系。方法:回顧性分析因單純輸卵管因素或盆腔粘連原因行第1周期體外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)治療的患者資料,排除已知的影響流產(chǎn)和胚胎發(fā)育疾病的患者,將患者按流產(chǎn)次數(shù)分為4組,分別為既往自然流產(chǎn)次數(shù)0次組(n=1 252)、1次組(n=385)、2次組(n=77)和≥3次組(n=36)。結(jié)果:第3日的優(yōu)質(zhì)胚胎率和受精率各組間比較均有統(tǒng)計(jì)學(xué)差異(P0.05);既往自然流產(chǎn)0次、1次、2次組的優(yōu)質(zhì)胚胎率隨流產(chǎn)次數(shù)增加呈下降趨勢(shì)(P=0.005);但既往自然流產(chǎn)≥3次組的優(yōu)質(zhì)胚胎率和受精率較2次自然流產(chǎn)組均有升高趨勢(shì),進(jìn)一步分析患者臨床資料顯示,既往自然流產(chǎn)≥3次的患者在IVF-ET前進(jìn)行全面的自然流產(chǎn)原因篩查和治療的比例顯著高于2次自然流產(chǎn)組,差異具有統(tǒng)計(jì)學(xué)意義(P=0.011)。結(jié)論:既往2次自然流產(chǎn)患者的胚胎質(zhì)量下降;經(jīng)系統(tǒng)、全面的自然流產(chǎn)原因篩查,并接受相應(yīng)治療后再行IVF-ET,自然流產(chǎn)次數(shù)≥3次的患者其受精率和優(yōu)質(zhì)胚胎率可有所提高。
