PI3K-AKT信號(hào)通路在宮頸病變中的表達(dá)及其與HPV感染關(guān)系的研究

【摘要】:目的:磷脂酰肌醇3-激酶(phosphoinositide 3-kinase,PI3K)及處于其下游的蛋白激酶B(protein kinase B,Akt或PKB)組成的信號(hào)通路在人類(lèi)腫瘤的發(fā)生發(fā)展演變過(guò)程中發(fā)揮重要作用,且現(xiàn)已證實(shí)宮頸癌的發(fā)生與人乳頭瘤病毒(Human Papilloma Virus,HPV)的持續(xù)性感染相關(guān)。本研究旨在探討PI3K、AKT在宮頸病變組織中的表達(dá)及其與HPV感染的相關(guān)性。方法:1、收集蚌埠醫(yī)學(xué)院第一附屬醫(yī)院子宮頸活檢及手術(shù)切除標(biāo)本174例。其中正常宮頸組織30例、宮頸上皮內(nèi)瘤變組織76例(LSIL組織18例,HSIL組織58例)、宮頸癌組織68例(I期36例、II期32例)。2、應(yīng)用免疫組化SP法染色檢測(cè)PI3K p110α、p-AKT蛋白在中的表達(dá)情況。3、同時(shí)應(yīng)用雜交捕獲(HC2)技術(shù)進(jìn)行宮頸粘液脫落細(xì)胞HPV-DNA檢測(cè)。4、采用SPSS17.0對(duì)數(shù)據(jù)進(jìn)行處理,結(jié)果結(jié)合臨床資料進(jìn)行分析,應(yīng)用c2檢驗(yàn),Mann-Whitney U檢驗(yàn),Spearman等級(jí)相關(guān)分析進(jìn)行統(tǒng)計(jì)。P0.05定義為有統(tǒng)計(jì)學(xué)差異。結(jié)果:1、PI3K p110α蛋白在宮頸癌組的表達(dá)明顯高于在LSIL、HSIL組(P0.05),其在HSIL組的表達(dá)也明顯高于LSIL組(P0.01)。p-Akt蛋白在LSIL的表達(dá)明顯低于在HSIL組及宮頸癌組中的表達(dá)(P0.01),而HSIL組與與宮頸癌組之間表達(dá)差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。2、PI3K p110α蛋白與p-Akt蛋白在病變宮頸組織中的表達(dá)具有明顯的正相關(guān)性(P0.001)。3、PI3K p110α蛋白與p-Akt蛋白表達(dá)與HPV感染在各宮頸組織中的表達(dá)具有明顯的正相關(guān)性(P0.05)。4、PI3K p110α、p-Akt的表達(dá)與宮頸癌FIGO分期、病理學(xué)分級(jí)以及淋巴結(jié)轉(zhuǎn)移無(wú)關(guān)(P0.05)。結(jié)論:PI3K p110α、p-Akt表達(dá)及HPV感染在宮頸病變過(guò)程中密切相關(guān),聯(lián)合檢測(cè)上述指標(biāo)可能為宮頸病變的早期診斷提供依據(jù),針對(duì)上述指標(biāo)的靶向藥物的應(yīng)用可能為宮頸癌的治療提供思路和方向。
[Abstract]:Objective: the signaling pathway composed of phosphoinositide 3-kinase PI3K and its downstream protein kinase B (protein kinase Bu Akt or PKB plays an important role in the development and development of human tumors. It has been confirmed that cervical cancer is associated with persistent infection of human papillomavirus (Human Papilloma). The purpose of this study was to investigate the expression of PI3 Knakt in cervical lesions and its correlation with HPV infection. Methods one hundred and forty four specimens of cervical biopsy and surgical excision were collected from the first affiliated Hospital of Bengbu Medical College. Among them, 30 cases were normal cervix tissues. There were 76 cases of cervical intraepithelial neoplasia (58 cases of LSIL) and 68 cases of cervical carcinoma (32 cases of stage I 36 cases of stage II) .2The expression of PI3K p110 偽 -p-AKT protein was detected by SP immunohistochemical staining, and the expression of p110 偽 -p-AKT protein was detected by hybridization capture. (HC2) technique was used to detect HPV-DNA of cervical mucus exfoliated cells. SPSS17.0 was used to process the data. Results according to the clinical data, using C2 test Mann-Whitney U test and Spearman grade correlation analysis, the statistical analysis was defined as statistical difference. Results the expression of PI3K p110 偽 protein in cervical cancer group was significantly higher than that in Lsil HSIL group (P0.05), and its expression in HSIL group was significantly higher than that in LSIL group (P0.01) .p-Akt protein expression in LSIL was significantly lower than that in HSIL group and cervical cancer group (P0.01), while that in HSIL group and cervical cancer group was significantly lower (P0.01). There was no significant difference between the expression of PI3K p110 偽 protein and p-Akt protein in cervical lesions (P0.05). (P0.001) .3PI3K p110 偽 protein and p-Akt protein expression and HPV infection in cervical tissues were significantly positive correlation (P0. 001). 3 PI3K p110 偽 protein expression and HPV infection in the cervix tissues, there was a significant positive correlation between the expression of PI3K p110 偽 protein and the expression of HPV protein in cervical tissues. The correlation between the expression of PI3K p110 偽 p-Akt and the FIGO stage of cervical cancer was observed. Pathological grading and lymph node metastasis were not significant (P0.05). Conclusion the expression of 10% PI3K p110 偽 p-Akt and HPV infection are closely related to the process of cervical lesions. The combined detection of these markers may provide evidence for the early diagnosis of cervical lesions. The application of targeted drugs for these indicators may provide ideas and directions for the treatment of cervical cancer.
