【摘要】：目的建立適合于在新疆醫(yī)科大學第一附屬醫(yī)院就診的妊娠期婦女血清甲狀腺激素水平參考值,為來本院就診的妊娠婦女甲狀腺疾病的早期診斷和治療提供臨床依據(jù)。方法隨訪2015年3月~2015年12月在新疆醫(yī)科大學第一附屬醫(yī)院就診的健康妊娠期婦女126人,采用電化學發(fā)光分析方法檢測其血清促甲狀腺激素(TSH)、游離三碘甲狀腺原氨酸(FT3)、游離甲狀腺素(FT4)、甲狀腺球蛋白抗體(Tg-Ab)和甲狀腺過氧化物酶抗體(TPO-Ab)的濃度,血清甲狀腺功能指標參考值制定方法采取美國臨床生化研究院(NACB)推薦的方法,建立妊娠各期甲狀腺激素水平參考范圍。結果隨訪126名健康妊娠期婦女,得到了其血清FT3,FT4,TSH,TPO-Ab,Tg-Ab的參考值范圍。其中血清FT3、FT4和TSH水平在妊娠各期之間的差異均有統(tǒng)計學意義(P0.001),與目前實驗室使用的正常非孕女性的參考值之間的差異有統(tǒng)計學意義。正常非孕女性與妊娠期婦女各期的血清TPO-Ab和Tg-Ab水平差異無統(tǒng)計學意義(P0.05)。結論獲得了新疆醫(yī)科大學第一附屬醫(yī)院妊娠期婦女甲狀腺激素參考值范圍。故各地區(qū)應建立本地區(qū)或本單位的妊娠期甲狀腺參考值范圍,以利于對妊娠婦女甲狀腺疾病進行篩查,利于早期診斷和治療。
[Abstract]:Objective to establish the reference value of serum thyroid hormone level of pregnant women in the first affiliated Hospital of Xinjiang Medical University, and to provide clinical basis for the early diagnosis and treatment of thyroid diseases of pregnant women. Methods A total of 126 healthy pregnant women were followed up from March 2015 to December 2015 in the first affiliated Hospital of Xinjiang Medical University. The concentrations of serum thyroid stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4), thyroglobulin antibody (Tg-Ab) and thyroid peroxidase antibody (TPO-Ab) were determined by electrochemiluminescence (ECL). The reference value of serum thyroid function index was established by the method recommended by the American Institute of Clinical Biochemistry (NACB) to establish the reference range of thyroid hormone level during pregnancy. Results 126 healthy pregnant women were followed up and the reference range of TPO-AbTg-Ab was obtained. There were significant differences in serum FT _ 3 FT _ 4 and TSH levels between pregnant women and normal non-pregnant women (P0. 001). There was no significant difference in serum TPO-Ab and Tg-Ab levels between normal nonpregnant women and pregnant women (P0.05). Conclusion the reference range of thyroid hormone in pregnant women in the first affiliated Hospital of Xinjiang Medical University was obtained. Therefore, the reference value range of thyroid gland in pregnancy should be established in each area or unit in order to screen thyroid diseases in pregnant women and to diagnose and treat thyroid diseases in early stage.
