【摘要】：目的探討同步放化療治療中晚期宮頸癌的臨床效果,分析宮頸癌并發(fā)局部感染的危險(xiǎn)因素。方法選擇2010年1月-2011年1月于醫(yī)院就診的中晚期宮頸癌患者60例,隨機(jī)分為觀察組和對(duì)照組,各30例,對(duì)照組給予單純放射治療,觀察組給予同步放化療治療,比較兩組近期療效及不良反應(yīng),隨訪5年,比較兩組近遠(yuǎn)期療效,分析感染相關(guān)影響因素。結(jié)果觀察組近期總有效率為93.33%顯著高于對(duì)照組的73.33%(P0.05),觀察組5年生存率為76.67%顯著高于對(duì)照組的53.33%(P0.05);觀察組局部復(fù)發(fā)率及遠(yuǎn)處轉(zhuǎn)移率分別為16.67%,13.33%顯著低于對(duì)照組33.33%,26.67%(P0.05);對(duì)照組不良反應(yīng)發(fā)生率13.33%,與觀察組23.33%差異無(wú)統(tǒng)計(jì)學(xué)意義。觀察組發(fā)生局部感染20例,感染率為66.67%;住院時(shí)間7d、合并有基礎(chǔ)疾病、醫(yī)護(hù)人員的手術(shù)不熟練、尿管留置時(shí)間3d、未應(yīng)用抗菌藥物、FIGO分期IVb期是宮頸癌患者腔內(nèi)放療并發(fā)局部感染的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論對(duì)中晚期宮頸癌患者采取同步放化療治療的療效較好,提高患者生存率,且耐受性較好。
[Abstract]:Objective to investigate the clinical effect of concurrent radiotherapy and chemotherapy in the treatment of advanced cervical cancer and to analyze the risk factors of cervical cancer complicated with local infection. Methods from January 2010 to January 2011, 60 patients with advanced cervical cancer were randomly divided into two groups: the observation group (n = 30) and the control group (n = 30). The control group was treated with radiotherapy alone, and the observation group was treated with concurrent radiotherapy and chemotherapy. The short term curative effect and adverse reaction were compared between the two groups, followed up for 5 years, the near and long term curative effect were compared, and the influencing factors of infection were analyzed. Results the short-term total effective rate of the observation group was 93.33% significantly higher than that of the control group 73.33% (P0.05), the 5-year survival rate of the observation group was 76.67% significantly higher than that of the control group 53.33% (P0.05), the local recurrence rate and distant metastasis rate of the observation group were 13.33% significantly lower than that of the control group (26.67%), and the control group was significantly lower than the control group (P0.05). The incidence of adverse reaction was 13.33%, and there was no significant difference between observation group and observation group (23.33%). In the observation group, 20 cases of local infection occurred, the infection rate was 66.67%, the hospital stay was 7 days, the patients were complicated with basic diseases, and the medical staff were not skilled in operation. Urinary catheter indwelling time 3 days, no use of antibiotics Figo stage IVb stage of cervical cancer patients with intracavitary radiotherapy associated with local infection of the independent risk factors (P0.05). Conclusion simultaneous radiotherapy and chemotherapy is effective in the treatment of advanced cervical cancer, and the survival rate is improved.
