【摘要】：目的探討分析米非司酮輔助COOK宮頸擴(kuò)張雙球囊在瘢痕子宮妊娠中期引產(chǎn)中的應(yīng)用效果。方法將該院2014年6月-2015年12月收治的瘢痕子宮妊娠中期需行引產(chǎn)患者78例作為研究對(duì)象,按照隨機(jī)數(shù)字表法遵循雙盲原則分為研究組和對(duì)照組,各39例。對(duì)照組患者給予常規(guī)方案引產(chǎn),研究組給予米非司酮輔助COOK宮頸擴(kuò)張雙球囊方案引產(chǎn)。觀(guān)察對(duì)比兩種治療方案臨床效果,并分析其應(yīng)用價(jià)值。結(jié)果兩組患者陰道引產(chǎn)率均為100%;兩組顯效、有效、無(wú)效病例等級(jí)比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),且前者總有效率為97.44%,較對(duì)照組的79.49%顯著升高,差異有統(tǒng)計(jì)學(xué)意義(P0.05);研究組規(guī)律宮縮時(shí)間、規(guī)律宮縮至胎盤(pán)娩出時(shí)間、引產(chǎn)總時(shí)間以及引產(chǎn)出血量均較對(duì)照組顯著減少,兩組間差異有統(tǒng)計(jì)學(xué)意義(P0.05);研究組引產(chǎn)后并發(fā)癥發(fā)生率為5.13%,對(duì)照組為23.08%,前者遠(yuǎn)低于后者,兩組間差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論在瘢痕子宮妊娠中期引產(chǎn)中采用米非司酮輔助COOK宮頸擴(kuò)張雙球囊療效顯著,不僅能夠有效縮短產(chǎn)程,減少出血量,還可有效減少引產(chǎn)術(shù)后并發(fā)癥情況,臨床推廣應(yīng)用價(jià)值較高。
[Abstract]:Objective to investigate the effect of mifepristone assisted double balloon dilatation of cervix in the second trimester of cicatricial uterus. Methods from June 2014 to December 2015, 78 patients with induced labor were divided into study group (39 cases) and control group (39 cases) according to double blind principle. The patients in the control group were given routine induction of labor, and the study group were given mifepristone to induce labor with double balloon regimen of COOK cervical dilatation. To observe and compare the clinical effects of two therapeutic schemes and to analyze their application value. Results the rate of vaginal induced labor in the two groups was 100. There was significant difference in the grade of effective and ineffective cases between the two groups (P0.05), and the total effective rate of the former was 97.44%, which was significantly higher than that of the control group (79.49%). The difference was statistically significant (P0.05); the time of regular uterine contraction, the time of regular uterine contraction to placenta delivery, the total time of induced labor and the amount of induced bleeding in the study group were significantly lower than those in the control group. There was significant difference between the two groups (P0.05); the incidence of complications after induced labor was 5.13 in the study group, 23.08 in the control group, the former was much lower than the latter, the difference between the two groups was statistically significant (P0.05). Conclusion Mifepristone combined with COOK for cervical dilatation is effective in the induction of labor in the second trimester of cicatricial uterus. It can not only effectively shorten the labor process and reduce the amount of bleeding, but also can effectively reduce the complications after induced labor. The value of clinical application is high.
