【摘要】：目的探討不同硬膜外分娩鎮(zhèn)痛方案對(duì)產(chǎn)后尿潴留的影響。方法選取2016年1月-2016年9月該院擬硬膜外分娩鎮(zhèn)痛下自然分娩產(chǎn)婦76例。按照數(shù)字表法隨機(jī)分為兩組:A組患者給予0.1%羅哌卡因+0.3μg/ml舒芬太尼,B組0.075%羅哌卡因+0.5μg/ml舒芬太尼。兩組患者均于宮口開放3 cm時(shí)經(jīng)L_3-L_4間隙行硬膜外穿刺置管行分娩鎮(zhèn)痛。記錄產(chǎn)婦行分娩鎮(zhèn)痛后視覺模擬評(píng)分(VAS)、下肢肌力、尿潴留及新生兒出生后Apgar評(píng)分。結(jié)果兩組產(chǎn)婦分娩鎮(zhèn)痛后各時(shí)間點(diǎn)VAS評(píng)分4,組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者均未出現(xiàn)明顯雙下肢肌力減退。兩組新生兒出生后Apgar評(píng)分差異無統(tǒng)計(jì)學(xué)意義(P0.05)。產(chǎn)后6 h A組出現(xiàn)2例產(chǎn)后尿潴留,B組出現(xiàn)8例,組間差異有統(tǒng)計(jì)學(xué)意義(P0.05)。產(chǎn)后24 h兩組患者均未發(fā)現(xiàn)明顯尿潴留。結(jié)論硬膜外給予0.1%羅哌卡因+0.3μg/ml舒芬太尼或0.075%羅哌卡因+0.5μg/ml舒芬太尼均可有效緩解產(chǎn)婦分娩疼痛,但前者產(chǎn)后尿潴留比例更低。
[Abstract]:Objective to investigate the effect of epidural analgesia on postpartum urinary retention. Methods 76 cases of spontaneous delivery from January 2016 to September 2016 were selected. According to the digital table method, two groups were randomly divided into two groups: group A received 0.1% ropivacaine 0.3 渭 g/ml sufentanil, group B 0.075% ropivacaine 0.5 渭 g/ml sufentanil. The patients in both groups were treated with epidural catheterization and labor analgesia through L_3-L_4 space 3 cm after the opening of uterine orifice. Visual analogue score (VAS), urinary retention and postnatal Apgar score were recorded. Results there was no significant difference in VAS score between the two groups at each time point after labor analgesia (P0.05). In both groups, there was no significant decrease in lower limb muscle strength. There was no significant difference in postnatal Apgar score between the two groups (P0.05). There were 2 cases of postpartum urinary retention in group A and 8 cases in group B at 6 hours postpartum, the difference was statistically significant (P0.05). No significant urinary retention was found in both groups at 24 h postpartum. Conclusion epidural administration of 0.1% ropivacaine 0.3 渭 g/ml sufentanil or 0.075% ropivacaine 0.5 渭 g/ml sufentanil can effectively relieve parturient pain, but the rate of postpartum urinary retention is lower in the former.
