【摘要】：目的探討B(tài)族鏈球菌感染和陰道微生態(tài)失調(diào)與胎膜早破的關(guān)系及其對(duì)妊娠結(jié)局的影響。方法選擇醫(yī)院2015年1月-2016年12月足月胎膜早破孕婦80例為胎膜早破組,足月未胎膜早破孕婦80例為對(duì)照組;觀(guān)察兩組孕婦下生殖道B族鏈球菌感染和陰道微生態(tài)結(jié)果。結(jié)果胎膜早破組陰道pH值4.5比例、B族鏈球菌感染率、微生態(tài)失調(diào)發(fā)生率為58.75%、17.50%、85.00%均高于對(duì)照組32.50%、3.75%、55.00%(P0.05),兩組陰道清潔度、外陰陰道假絲酵母菌陰道炎、細(xì)菌性陰道病比較差異無(wú)統(tǒng)計(jì)學(xué)意義;80例胎膜早破患者中,B族鏈球菌陽(yáng)性患者絨毛膜羊膜炎、產(chǎn)褥感染、胎兒窘迫和新生兒肺炎的發(fā)生率為35.71%、35.71%、42.86%、28.57%均高于B族鏈球菌陰性患者6.06%、7.58%、7.58%、4.55%(P0.05);微生態(tài)失衡患者和微生態(tài)正�；颊呓q毛膜羊膜炎、產(chǎn)褥感染、胎兒窘迫、新生兒肺炎比較差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論足月胎膜早破孕婦B族鏈球菌感染增加絨毛膜羊膜炎、產(chǎn)褥感染、胎兒窘迫和新生兒肺炎的發(fā)生率,陰道微生態(tài)失調(diào)對(duì)妊娠結(jié)局影響較小。
[Abstract]:Objective to investigate the relationship between group B streptococcus infection and vaginal microecological disorder and premature rupture of fetal membrane and its effect on pregnancy outcome. Methods from January 2015 to December 2016, 80 pregnant women with premature rupture of membranes were selected as the group of premature rupture of membranes and 80 cases as control group, and the results of B streptococcus infection and vaginal microecology in the lower genital tract of pregnant women were observed. Results the vaginal pH value of premature rupture of membranes group was 4.5, the incidence of microecologic disorder was 58.75 and 17.50%, and 85.00% was higher than that of the control group (P 0.05). The vaginal cleanliness, vulvovaginal candida vaginitis and vaginitis of the two groups were significantly higher than that of the control group (P 0.05). There was no significant difference in bacterial vaginosis. In 80 patients with premature rupture of membranes, chorioamnionitis and puerperal infection were found in group B streptococcus positive patients. The incidence of fetal distress and neonatal pneumonia was 35.71% and 28.57% higher than that of group B streptococcus negative patients (P 0.05), while there was no significant difference between microecological unbalance patients and microecological normal patients with chorioamnionitis, puerperal infection, fetal distress and neonatal pneumonia. Conclusion the incidence of chorioamnionitis, puerperal infection, fetal distress and neonatal pneumonia were increased in pregnant women with premature rupture of membranes.
