胎盤中IGF-1、IGF-2、IGF-1R以及IGFBP-3表達水平與妊娠期糖尿病巨大兒的相關(guān)性研究

【摘要】:目的:分別測定分娩巨大兒的妊娠期糖尿。℅DM)孕婦的胎盤組織和分娩巨大兒的正常孕婦胎盤組織中胰島素樣生長因子-1(IGF-1)、胰島素樣生長因子-2(IGF-2)、胰島素樣生長因子-1受體(IGF-1R)及胰島素樣生長因子結(jié)合蛋白-3(IGFBP-3)的mRNA表達水平,探討IGF-1、IGF-2、IGF-1R及IGFBP-3在妊娠期糖尿病巨大兒形成中的作用,為研究妊娠期糖尿病巨大兒發(fā)病機制提供理論依據(jù),同時也為臨床妊娠期糖尿病的治療和預(yù)防巨大兒的產(chǎn)生提供理論支持。 資料與方法: 1、資料來源 1.1GDM組:選擇2010年1月至2012年12月在中國人民解放軍306醫(yī)院產(chǎn)前檢查、孕期確診妊娠期糖尿病且分娩巨大兒的初產(chǎn)婦胎盤22例,,病例入選條件:(1)孕母既往無糖尿病史,于孕24-28周行75gOGTT試驗,結(jié)果判定參照2011年美國糖尿病學(xué)會(ADA)診斷標(biāo)準(zhǔn),血糖水平滿足以下之一者:空腹血糖≥5.1mmol/l,服糖后1小時血糖≥10.0mmol/l,服糖后2小時血糖≥8.5mmol/l,均為首次妊娠,無妊娠期高血壓病、妊娠期甲狀腺功能異常等其他妊娠期并發(fā)癥,同時無家族遺傳病史(2)新生兒出生體重≥4000g,且無先天畸形,Apgar評分均為7-10分。 1.2正常對照組:選擇同期在本院住院并分娩、無妊娠期糖尿病且分娩巨大兒的初產(chǎn)婦胎盤組織18例,病例入選條件:(1)所有產(chǎn)婦均為初次妊娠,既往無孕產(chǎn)史,孕期糖耐量試驗正常,無其他任何妊娠期合并癥,且無家族性遺傳性疾。唬2)新生兒無先天畸形,出生體重≥4000g之間,Apgar評分均為7-10分。 2、方法 2.1標(biāo)本采集及處理符合上述條件的孕婦在胎兒娩出后,均在分娩或剖宮產(chǎn)時于胎盤娩出后5分鐘內(nèi)從胎盤母體面中央(避開壞死及鈣化區(qū))取絨毛組織塊約1cm×1cm×1cm,生理鹽水溶液充分漂洗后置入凍存管封存,液氮中保存以待mRNA的提取。 2.2胎盤組織中胰島素樣生長因子-1(IGF-1)、胰島素樣生長因子2(IGF-2)、胰島素樣生長因子-1受體(IGF-1R)及胰島素樣生長因子結(jié)合蛋白-3(IGFBP-3)mRNA檢測采用Real Time PCR技術(shù)測定妊娠期糖尿病并分娩巨大兒產(chǎn)婦胎盤組織及正常孕婦分娩巨大兒胎盤組織中胰島素樣生長因子受體I(IGF-1R)以及IGF結(jié)合蛋白3(IGFBP-3)的mRNA水平。 結(jié)果 1、GDM組患者胎盤中IGF-1、IGF-2、IGF-1R的表達量要顯著高于正常對照組,差異有統(tǒng)計學(xué)意義(P0.05)。 2、在妊娠糖尿病患者胎盤中IGFBP-3的表達水平較正常對照組低,差異有統(tǒng)計學(xué)意義(P0.05)。 結(jié)論 1、提示GDM孕婦巨大兒的分娩可能與胎盤組織中IGF-1、IGF-2及IGF-1R表達水平的升高有密切關(guān)系; 2、GDM組胎盤組織中IGFBP-3表達水平的降低可能是導(dǎo)致GDM孕婦巨大兒發(fā)生的重要原因之一。
[Abstract]:Objective: to determine the levels of insulin-like growth factor-1 (IGF-1), insulin-like growth factor-2 (IGF-2) and insulin-like growth factor-1 (IGF-1) in placenta of pregnant women with gestational diabetes mellitus (GDM) and normal pregnant women with macrosomia. The expression of mRNA in receptor (IGF-1R) and insulin-like growth factor binding protein 3 (IGFBP-3), To explore the role of IGF-1R and IGFBP-3 in the formation of gestational diabetic macrosomia, to provide theoretical basis for studying the pathogenesis of gestational diabetic macrosomia, and to provide theoretical support for clinical treatment of gestational diabetes mellitus and prevention of macrosomia.
