子癇前期相關高危因素的臨床研究

【摘要】:目的分析子癇前期與正常孕婦相關指標的差異,探討子癇前期發(fā)病的高危因素。方法選取2004年4月至2014年4月產科住院的子癇前期產婦2 180例作為研究組,選擇同期住院正常產婦2 980例作為對照組。收集2組孕婦年齡、孕前體重指數(BMI)、孕期增重、入組時的血壓、空腹血糖(FPG)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、載脂蛋白-A1(Apo A1)和載脂蛋白-B(Apo B)進行分析。結果研究組孕前BMI、孕期增重分別為(25.11±4.03)kg/m2和(19.28±3.36)kg,與對照組(21.27±2.43)kg/m2和(14.52±2.33)kg比較,差異有統(tǒng)計學意義(P0.05)。研究組孕婦入組時收縮壓、舒張壓分別為(147.25±14.22)mm Hg和(100.55±10.98)mm Hg,對照組分別為(109.65±11.33)mm Hg和(69.51±8.06)mm Hg,2組比較,差異均有統(tǒng)計學意義(P0.01)。研究組FPG、TG和Apo B分別為(4.95±1.98)mmol/L、(4.42±1.83)mmol/L和(1.18±0.38)g/L,對照組分別為(3.54±1.03)mmol/L、(3.33±1.22)mmol/L和(1.01±0.22)g/L,兩2組比較,差異均有統(tǒng)計學意義(P0.05)。logistic回歸分析顯示:孕前BMI(OR值9.85)、孕期增重(OR值5.17)、FPG(OR值6.48)、TG(OR值6.45)、Apo B(OR值6.98)、年齡(OR值5.58)及平均動脈壓(OR值5.27)均為子癇前期發(fā)病的危險因素(P0.05)。結論子癇前期的發(fā)病與多因素相關,其中孕前BMI、孕期增重、FPG、TG、Apo B、年齡及平均動脈壓是罹患子癇前期的危險因素。
[Abstract]:Objective to study the high risk factors of preeclampsia by analyzing the difference between preeclampsia and normal pregnant women. Methods from April 2004 to April 2014, 2 180 cases of preeclampsia in obstetrics department were selected as study group and 2 980 cases of normal pregnant women in the same period as control group. Age, body mass index (BMI),) during pregnancy, blood pressure, fasting blood glucose, (FPG), triglyceride (TG), low density lipoprotein (LDL-C), high density lipoprotein (HDL-C), apolipoprotein A 1 (Apo A1) and apolipoprotein B (Apo B) were analyzed. Results the weight gain during pregnancy was (25.11 鹵4.03) kg/m2 and (19.28 鹵3.36) kg in the study group, which was significantly higher than that in the control group (21.27 鹵2.43) kg/m2 and (14.52 鹵2.33) kg (P0.05). Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were (147.25 鹵14.22) mm Hg) and (100.55 鹵10.98) mm Hg,) in the study group (109.65 鹵11.33) mm Hg and (69.51 鹵8.06) mm), respectively. There were significant differences between the two groups (P0.01). The TG and Apo B of the study group were (4.95 鹵1.98) mmol / L, (4.42 鹵1.83) mmol/L and (1.18 鹵0.38) g / L, respectively, and those of the control group were (3.54 鹵1.03) mmol / L, (3.33 鹵1.22) mmol/L and (1.01 鹵0.22) g / L, respectively. The differences were statistically significant (P0.05). Logistic regression analysis showed that BMI (OR before pregnancy (OR 5.17), weight gain during pregnancy (OR 6.48), TG (OR 6.45) and Apo B (OR (6.98), age (OR 5.58) and mean arterial pressure (OR 5.27) were the risk factors of preeclampsia (P0.05). Conclusion the incidence of preeclampsia is related to many factors, among which BMI before pregnancy, FPGG TGG Apo B during pregnancy, age and mean arterial pressure are risk factors of preeclampsia.
