【摘要】：目的探討腹腔鏡輔助經(jīng)臍入路治療小兒卵巢囊腫蒂扭轉(zhuǎn)的臨床價(jià)值。方法 2012年3月~2015年3月對6例小兒卵巢囊腫蒂扭轉(zhuǎn)行急診腹腔鏡輔助經(jīng)臍入路探查、卵巢囊腫切除術(shù),術(shù)中行囊液穿刺減壓后,經(jīng)臍擴(kuò)大切口將囊腫提出腹腔外行囊腫剝除術(shù)。結(jié)果 6例均順利完成手術(shù),成功保留患側(cè)卵巢,手術(shù)時(shí)間45~90 min,(50±22)min;術(shù)后住院3~5 d,平均4 d。6例隨訪6~12個(gè)月,平均10個(gè)月,腹部B超檢查無復(fù)發(fā),切口瘢痕小,臍部外觀滿意。結(jié)論腹腔鏡輔助經(jīng)臍入路手術(shù)具有創(chuàng)傷小、恢復(fù)快、操作簡單、易推廣等優(yōu)點(diǎn),對于腹腔鏡手術(shù)操作困難的患者是一種較好選擇。
[Abstract]:Objective to evaluate the clinical value of laparoscopic assisted umbilical approach in the treatment of ovarian cyst pedicle torsion in children. Methods from March 2012 to March 2015, 6 children with ovarian cyst pedicle torsion underwent laparoscopic assisted transumbilical approach exploration, ovarian cyst resection, and puncture and decompression of cystic fluid during the operation. Extraperitoneal excision of cysts was performed through an enlarged incision of the umbilical cord. Results the operation was successfully completed in all 6 cases and the affected ovary was preserved successfully. The operative time was 4590 minutes (50 鹵22) mins, and the postoperative hospitalization was 3 ~ 5 days, with an average of 4 days. 6 cases were followed up for 6 ~ 12 months (mean 10 months). No recurrence was found in abdominal B-ultrasound examination, the incision scar was small, and the appearance of umbilical cord was satisfactory. Conclusion Laparoscopic assisted transumbilical approach has the advantages of less trauma, quick recovery, simple operation and easy to be popularized. It is a good choice for patients with difficult operation of laparoscopic surgery.
