【摘要】：目的探討運用PRWY陰道鏡智能診斷評估技術(PRWY系統(tǒng))正確識別正常陰道鏡所見(NCF)和宮頸低級別鱗狀上皮內病變(LSIL)的原則。方法選取2013年9月至2014年8月北京大學第一醫(yī)院婦產科601例經陰道鏡檢查后組織病理診斷為慢性宮頸炎或LSIL的病例,描述PRWY系統(tǒng)的5個指標中NCF和LSIL的分布情況;利用卡方獨立性檢驗分析5個評估指標、年齡、薄層液基細胞學(TCT)的結果與組織病理學結果有無關聯;以LSIL的漏診率在20%以內為可接受范圍,結合年齡與TCT結果判別NCF和LSIL。結果根據PRWY系統(tǒng),陰道鏡下指標"紅紅黃"、"紅紅淺棕"、"紅白淺棕"中NCF的比例為64.10%、82.98%、73.28%;"粉白黃"和"粉白淺棕"中LSIL的比例為53.64%和46.15%;"紅紅淺棕"、"紅白淺棕"且年齡≥50歲、"紅紅黃"且TCT正常的患者中NCF所占的比例分別為82.98%、95.24%和87.50%,判定為NCF,其他條件下判定為LSIL。結論通過PRWY系統(tǒng)的5個評估指標,同時結合年齡和TCT的結果,可判別NCF和LSIL。
[Abstract]:Objective to explore the principle of correct identification of normal colposcopy (NCF) and cervical low grade squamous intraepithelial lesions (lsil) by using PRWY colposcopy intelligent diagnosis and evaluation technique (PRWY system). Methods from September 2013 to August 2014, 601 cases of gynecology and obstetrics department in the first Hospital of Peking University were selected, and the distribution of NCF and lsil in the five indexes of prwy system were described. Chi-square independence test was used to analyze five evaluation indexes, age, TCT results and histopathological results, and NCF and LSILs were judged with age and TCT results, taking the missed diagnosis rate of lsil within 20% as the acceptable range.
