【摘要】：目的探討改良B-Lynch縫合術(shù)在防治產(chǎn)后出血方面的臨床療效。方法選取2010年12月-2015年12月在該院產(chǎn)科行剖宮產(chǎn)手術(shù)的具有產(chǎn)后出血高危因素的80例患者。其中40例在剖宮產(chǎn)手術(shù)過(guò)程中行改良B-Lynch縫合術(shù)預(yù)防產(chǎn)后出血的患者設(shè)為觀察組,另40例在剖宮產(chǎn)手術(shù)過(guò)程中采用常規(guī)子宮按摩、宮腔填塞紗條和子宮腔球囊壓迫處理預(yù)防產(chǎn)后出血的患者設(shè)為對(duì)照組。比較兩組患者的手術(shù)時(shí)間、術(shù)中出血量、產(chǎn)后出血量以及術(shù)后恢復(fù)情況。結(jié)果觀察組患者的手術(shù)時(shí)間、術(shù)中出血量、術(shù)后24 h總出血量以及術(shù)后血紅蛋白的減少量均低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(t=2.660,2.653,3.709和7.386,P=0.009,0.010,0.001和0.001);兩組患者術(shù)后產(chǎn)褥病、宮腔黏連的發(fā)生率以及月經(jīng)復(fù)潮時(shí)間的比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);研究組術(shù)后血性惡露持續(xù)時(shí)間以及晚期產(chǎn)后出血發(fā)生率均低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(t=2.505,P=0.014;χ2=21.004,P=0.001)。結(jié)論改良B-Lynch縫合術(shù)操作簡(jiǎn)單,無(wú)需特殊器械和手術(shù)技巧,對(duì)于產(chǎn)后出血孕產(chǎn)婦具有顯著而又快速的止血效果,且術(shù)中損傷小、術(shù)后并發(fā)癥發(fā)生率低,值得臨床推廣。
[Abstract]:Objective to investigate the clinical effect of modified B-Lynch suture in the prevention and treatment of postpartum hemorrhage. Methods 80 patients with high risk factors of postpartum hemorrhage who underwent cesarean section from December 2010 to December 2015 were selected. Forty patients who underwent modified B-Lynch suture during cesarean section to prevent postpartum hemorrhage were set up as observation group, and 40 patients were treated with routine uterine massage during cesarean section. The patients with uterine cavity packing gauze and uterine cavity balloon compression to prevent postpartum hemorrhage were set up as control group. The operative time, intraoperative bleeding, postpartum bleeding and postoperative recovery were compared between the two groups. Results the time of operation, the amount of intraoperative bleeding, the total amount of blood loss 24 hours after operation and the decrease of hemoglobin in the observation group were lower than those in the control group, the difference was statistically significant (t = 2.660 / 2.653 / 3.709 and 7.386P _ (0.009) 0.010 ~ (-1), the puerperal diseases after operation were found in the two groups. There was no significant difference in the incidence of intrauterine adhesion and menstrual menorrhagia (P0.05), and the duration of hemorrhagic lochia and the incidence of late postpartum hemorrhage in the study group were lower than those in the control group (t = 2.505, P < 0.014; 蠂 ~ 2 = 21.004, P < 0.001). Conclusion the modified B-Lynch suture is simple, without special instruments and surgical techniques. It has a remarkable and rapid hemostatic effect for postpartum hemorrhage pregnant and parturient women, and has little injury during operation and low incidence of postoperative complications, so it is worth popularizing in clinic.
