【摘要】：目的探討負壓吸宮術治療剖宮產(chǎn)瘢痕部位妊娠(CSP)的可行性。方法回顧性分析2013年1月-2016年6月于該院住院治療且采用負壓吸宮術治療的69例CSP患者的臨床資料,手術順利為吸宮成功組(57例),術中出血多、紗布宮腔填塞、肌肉注射欣母沛或放置Foley氏球囊尿管、無效則中轉開腹手術或介入術,為吸宮失敗組(12例);分析兩組患者臨床療效及臨床預后。結果 69例患者中有57例患者治療成功,吸宮成功組中瘢痕處有殘留者27例;負壓吸宮術治療CSP的成功率約為82.6%,失敗率為17.4%。結論停經(jīng)時間短是負壓吸宮術成功治療CSP的關鍵,減少介入手術或外科手術等多重醫(yī)源性傷害,減輕患者的心理壓力及經(jīng)濟負擔,但要避免盲目吸宮;需要做好CSP的三級預防。
[Abstract]:Objective to explore the feasibility of negative pressure uterine suction for cesarean scar pregnancy (CSP). Methods the clinical data of 69 CSP patients who were hospitalized in our hospital from January 2013 to June 2016 were analyzed retrospectively. The patients who received intramuscular injection of Xinmupai or placed Foley's balloon urinary catheter were converted to open surgery or interventional surgery (12 cases). The clinical effect and prognosis of the two groups were analyzed. Results 57 of 69 patients were successfully treated, 27 of them had residual scar in the successful uterine suction group, and the success rate of the treatment of CSP was about 82.6, and the failure rate was 17.4%. Conclusion the short period of menopause is the key to the successful treatment of CSP by negative pressure aspiration, reducing multiple iatrogenic injuries such as interventional surgery or surgery, reducing the psychological pressure and economic burden of patients, but avoiding blind aspiration, and the third level prevention of CSP should be done well.
