【摘要】：目的探討腹腔鏡下垂體后葉素聯(lián)合病灶清除術(shù)與楔形切除術(shù)治療輸卵管間質(zhì)部妊娠的臨床效果。方法該院2011年1月-2013年6月輸卵管間質(zhì)部妊娠47例,根據(jù)患者的生育要求及術(shù)前血β-h CG情況選擇手術(shù)方式。研究組(垂體后葉素聯(lián)合病灶清除術(shù))22例,腹腔鏡下先在病灶周圍注射垂體后葉素6 u加生理鹽水10 ml,再切開病灶隆起部位,吸出孕囊及附屬組織,可吸收線縫合切口;對(duì)照組(楔形切除術(shù))25例,腹腔鏡下楔形切除病灶,可吸收線縫合切口。觀察手術(shù)時(shí)間、術(shù)中出血量、術(shù)后肛門排氣時(shí)間、術(shù)后住院時(shí)間和術(shù)后血β-HCG值恢復(fù)正常時(shí)間。結(jié)果兩組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后肛門排氣時(shí)間、術(shù)后住院時(shí)間和術(shù)后血β-HCG值恢復(fù)正常時(shí)間比較差異均無顯著性(P0.05);兩組均無1例持續(xù)性異位妊娠發(fā)生。結(jié)論腹腔鏡下垂體后葉素聯(lián)合病灶清除術(shù)治療輸卵管間質(zhì)部妊娠在徹底清除妊娠病灶的同時(shí),保留了患者子宮和輸卵管的完整性,適用于有生育要求的年輕患者。
[Abstract]:Objective to investigate the clinical effect of laparoscopic posterior pituitary lobectomy combined with wedge resection in the treatment of tubal interstitial pregnancy. Methods 47 cases of tubal interstitial pregnancy from January 2011 to June 2013 were selected according to their fertility requirements and preoperative blood 尾 -hCG. In the study group (22 cases), hypophyllin 6 u and normal saline 10 ml were injected around the lesion under laparoscope, then the protuberance of the lesion was cut out, the pregnancy sac and its subsidiary tissue were sucked out, and the sutured incision was sutured with absorbable line. The control group (25 cases) underwent laparoscopic wedge resection with absorbable suture incision. The time of operation, the amount of blood lost during operation, the time of anal exhaust after operation, the time of hospitalization after operation and the recovery time of serum 尾 -HCG were observed. Results there was no significant difference in operation time, blood loss, postoperative anal exhaust time, postoperative hospitalization time and recovery time of serum 尾 -HCG between the two groups (P0.05), and there was no one case of persistent ectopic pregnancy in both groups. Conclusion Laparoscopic pituitrin combined with focal debridement in the treatment of tubal interstitial pregnancy not only completely clears the focus of pregnancy, but also preserves the integrity of uterus and fallopian tube, which is suitable for young patients with fertility requirements.
