【摘要】：目的探討彩色多普勒超聲檢測(cè)大腦中動(dòng)脈(MCA)和臍動(dòng)脈(UA)阻力指標(biāo)對(duì)胎兒宮內(nèi)缺氧的診斷價(jià)值。方法選取該院臨床確診的胎兒宮內(nèi)缺氧孕婦121例,正常單胎無(wú)并發(fā)癥孕婦200例,利用彩色多普勒超聲檢查胎兒MCA及UA阻力指標(biāo),比較分析其敏感性、準(zhǔn)確性。結(jié)果與對(duì)照組比較,缺氧組胎兒MCA各阻力指標(biāo)均明顯降低,而UA各阻力指標(biāo)顯著升高,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);缺氧組MCA與UA各阻力指標(biāo)比值較對(duì)照組均明顯降低,差異有統(tǒng)計(jì)學(xué)意義(P0.01);MCA聯(lián)合UA評(píng)估胎兒宮內(nèi)缺氧靈敏度、準(zhǔn)確率高,漏診率低,而單以MCA較UA評(píng)估胎兒宮內(nèi)缺氧靈敏度、準(zhǔn)確率高,漏診率低。結(jié)論聯(lián)合檢測(cè)MCA及UA阻力指標(biāo)能夠較準(zhǔn)確預(yù)測(cè)胎兒宮內(nèi)缺氧,且較單項(xiàng)指標(biāo)靈敏度高、準(zhǔn)確率高,漏診率低。
[Abstract]:Objective to evaluate the diagnostic value of color Doppler ultrasonography in the diagnosis of fetal intrauterine hypoxia by detecting the resistance of middle cerebral artery (MCA) and umbilical artery (UA). Methods 121 cases of fetal intrauterine hypoxia and 200 cases of normal single fetus without complications were selected. The resistance indexes of fetal MCA and UA were examined by color Doppler ultrasound, and their sensitivity and accuracy were compared and analyzed. Results compared with the control group, the resistance index of MCA in hypoxia group decreased significantly, while the resistance index of UA increased significantly (P0.05), the ratio of resistance index of MCA to UA in hypoxia group was significantly lower than that in control group. The difference was statistically significant (P0.01). The sensitivity of intrauterine anoxia was higher and the rate of missed diagnosis was lower than that of MCA alone. The accuracy rate and missed diagnosis rate of MCA alone were higher than those of UA. Conclusion combined detection of MCA and UA resistance indexes can accurately predict fetal intrauterine hypoxia, and has higher sensitivity, higher accuracy and lower missed diagnosis rate than single index.
