【摘要】：目的:通過與CA125比較,探討外周血KISS1 mRNA在上皮性卵巢癌(epithelial ovarian cancer,EOC)中的診斷價(jià)值。方法:采集2010年1月至2014年1月在南通大學(xué)第二附屬醫(yī)院住院擬手術(shù)的EOC患者40例手術(shù)前一天的外周靜脈血和正常健康者靜脈血,用RT-PCR法和電化學(xué)發(fā)光法分別檢測(cè)外周血KISS1 mRNA和CA125的表達(dá)。結(jié)果:EOC早期組(Ⅰ~Ⅱ期)、晚期組(Ⅲ~Ⅳ期)KISS1 mRNA的表達(dá)量均顯著高于正常健康者(均P0.01),而早期與晚期EOC組間比較無顯著差異(P0.05);EOC晚期組CA125的表達(dá)量明顯高于早期組及正常健康者(均P0.01),而早期組與正常健康者比較無顯著差異(P0.05)。KISS1 mRNA ROC(receptive operator character curve,ROC)曲線的切值(cutoff)設(shè)為0.51、0.72時(shí),陽性預(yù)測(cè)值(positive predictive value,PPV)分別為0.58和1,陰性預(yù)測(cè)值(negative predictive value,NPV)分別為0.92和0.7;CA125的cutoff值設(shè)為20、100 U/ml時(shí),PPV分別為0.72和1,NPV分別為0.87和0.81。KISS1 mRNA、CA125對(duì)EOC均具有中度診斷效能(P=0.34);兩者比較,KISS1 mRNA對(duì)早期EOC具有較高診斷價(jià)值(P=0.018),而CA125對(duì)晚期EOC的診斷價(jià)值較高(P0.01)。結(jié)論:患者外周血KISS1 mRNA與CA125對(duì)EOC具有同等的診斷價(jià)值,KISS1 mRNA可作為EOC的一個(gè)新的腫瘤標(biāo)志物;兩者的聯(lián)合檢測(cè)可提高EOC的診斷效能,特別是早期EOC的診斷率。
